Lungs Initial measure for COPD patient!

In a patient with COPD which of the following is recommended initially?
A. phelebotomy in patient with 50% hematocrit
B. intermittent positive pressure
C. exercise with bicycle
D. nasal oxygen therapy with PaO2=58 mmHg

the only two measures shown to benefit a COPD patient mortality wise is home oxygen at Pao2 < 58 and smoking cessation (obviously at any time).

Correct Answer D

You always start with smoking cessation and home O2 if their is hypoxia.

Criteria for home O2:

1. Pao2<55mmHg or SaO2<88%

2. Pao2<60 if
  • Pulmonary HTN
  • Hematocrit >55%
3. Evidence of cor pulmonale

The ans is D
