Default Confirmatory testing vs. initiating treatment??

Guys, is there a general principle to follow when it comes to questions where the presentation is clear and the question asks about the best next step. When do you CONFIRM a diagnosis and when do you START treatment??

Please someone who had this problem help me out here. Thanks.

The general principal is if a patient is emergent then you go to treatment next. If it can wait and the presentation is not too obvious you test. Otherwise there is not any other general principal here. Different diseases require different approach and you need to learn it individually. E.g you should know when to immediately start treatment for pulmonary embolism and when to do more tests before you start treatment and like the one involving testicular cancer where you do orchiectomy before other time consuming modalities.
