how to ask about polyuria and lightheadedness in non-medical term question ?

Polyuria: excessive urination -or- "do you go the bathroom to urinate more often than usual?" -or- "do you wake up in the middle of the night to urinate?".

Lightheadedness: I think it's a common word, and patients can understand it, so you can use it as-is; "do you have lightheadedness?" -or- "do you feel sometimes as if you're about to fall?".
