Before asking the past medical history, you should use an appropriate transition "Thank you sir, I would now like to ask you questions related to your health in the past, as they might have a bearing on the current clinical condition"
This should be followed by an open ended question (beneficial in the CIS component) - "Can you tell me about your health in the past?"
The patient will tell you about any medical history here like DM, HTN, etc
This should be followed by three very pertinent questions
"Have you had similar episodes in the past?"
"Have you ever been hospitalized in the past?"
" Have you had any surgeries in the past?"
