Question Patient with psychosis

I am a little confused on how to take the history from a patient with psychosis..
How to word questions inquiring about all the positive and negative symptoms, disorganized speech and thought patterns etc.

Hallucinations
Schizophrenia: >=2 from hallucinations, delusions, disorganized speech, alogia, flat affect, asociality, apathy, inattention

How long have you been feeling this way?
Did anything particular happen before this started?
Is this constant or does it come and go?
What time of the day are your Voices the worst?
How does this affect your life? Your job? (Intensity)
Have you abandoned your contacts with friends? Family? (Asociality)

Hallucinations (Do you hear or see things that other people don’t?)
Do you have any other experience?
Do you see anything unusual?
Do you hear anything unusual?
Do you smell anything unusual?
Do you feel like bugs crawling on you?

Content
• What do they write? What do they say?
• Do you feel like being controlled by them?
• Do they order you to harm yourself? To harm other people?
• Do you think about harming yourself? Harming other people?

Sensory
Do you have any problems with your vision, besides…? Vision loss?
Do you have any problems with your hearing, besides…? Hearing loss?

Etiology
Have you had any recent trauma to your head?
Did you ever lose your consciousness? Have you had any abnormal jerking movements?
Do you have headache? N? V? Do you feel Weakness anywhere? Numbness?

Associated Symptoms
Thyroid
Do you feel cold or hot when others don’t? Any recent changes with your skin? Hair?
Sleep
How is your sleep?
Do you fall asleep suddenly during the day?

Depression/Mania
How is your mood?
Do you feel sad or depressed?
Do you have any hobbies and interests? Do you enjoy them now?
Do you meet your family and friends? (asocial)
How is your energy?
Do you feel elated?
Do you have racing thoughts?

FH Do you have family history of mental disorders??

SIGJ M CAPS

S- Speech
I- Insight
G- Grooming/ Appearance
J- Judgment
M- Mood
C- (COMA: Concentration, Orientation, Memory, Attention)
A- Affect
P- Perception (PhD: Paranoia, Hallucination, Delusion)
S- Suicidal/Homicidal ideation

Can you plz tell me whether to do this mental status exam in all psych cases? and do we write these in Physical exam part of PN?

Theoretically speaking YES . Practically depends on the case and how much time will have for Physical exam .

Mental status is the physical exam in Psychiatry .

right. N in CSE video they showed hallucination as only h/o case.Can this be possible in real exam also? i guess in that case MSE wd only be the examination to be done.

Again , Everything is possible in real exam .This has to be in the back of the mind all the time .

What is shown in the videos is 99% reviewed and should be correct .

MS should be in any neuro/psych case
