Help Summarizing and Closing

Hi,

My summary and closing seems unorganized and not fluid. Any tips on how to make it structured? So with each case I can follow the same format for summary and closing but make the necessary modifications for each case? Thanks.

Well, this is what I am doing. See if it helps you.

I summarize usually after I am done with the history taking, just before P/E. And I dont summarize everything. I think the SP's are looking if you do "SOME" kind of summarization. So what i say is "So let me just summarize what you have told me, so that if I've missed out something, you can correct me" and then I just say the most important aspects of HPI (no need to summarize PAMHUGSFOSS). Then I saw "Is there anything else you would like to tell me?"

Closing, I follow this method.
-"Thank you for the P/E and Hx. Based on my examination, I think you might be having a <1st diagnosis>. However, I am not sure. It could also be a <2nd> or <3rd>"

-You can also say "Thank you for the P/E and Hx. Based on my examination, I am considering a few possibilities. It could either be <1st> or <2nd> or <3rd>" I am making it a habit to stick to 2 or 3 MAX. Its pointless to give more diagnosis, since you also will be wasting time in explaining them. I.e you just cant say "Peptic Ulcer"..you have to explain it in layman's term "like an ulcer in your stomach area) etc

-After the diagnosis, I say "I would be doing some tests"....and explain them in layman's terms as well.

-Then, counsel them "In the mean time, as your physician, I am concerned about your health....",and counsel on Smoking/alcohol/blah blah.

-Lastly "Do you have anymore questions for me". If they say no, "Thank you for your time. I will get back to you as soon as I get your results, and then we will discuss further your treatment options or workup. It was nice seeing you. Have a great day"

What do you think? If anyone feels the need, please correct my mistakes or point out whats wrong.

Thanks so much for that. That sounds great. I will try to mold my summary and closure in a similar fashion. Perhaps those with more experience can weigh in. When is your test by the way?

Personally... I counsel on the way (kind of CSE videos way):

If they ETOH/TOBACCO/DRUGS --> I'd like to have the opportunity to mention: XY is very dangerous to you, can harm your XY, and you ABD. I'd feel better if you stop using X. Would you like to quit?
