Stethoscope Rapport and Professionalism in CS Exam

can anyone tell me some measures to pass..
Rapport and Professionalism,
plz add to following..

1- ask for feelings..and concerns one or two times in interview..
2- address the patient by last name..at least three times during interview
3- show empathy...where ever necessary..
4- ask the patient if he has some questions

are the above enough or some changes need to be made....

Smile Few more points

5- do not interrupt the patient when he/she talks
6- always ask for permission to do the examination
7- always show that you are concerned
8- smile when you first enter the room
9- keep adequate distance (do not go into the patient's face)
10- warm up the stethoscope before auscultation

Show Teeth Even more

11- if the patient cries give tissues
12- give the bad news in professional assertive style
13- shake hands in the beginning
14- never forget to wash your hands
15- always say "That must be very difficult for you, that must be very painful ...etc I will try my best to help you"
16- do not change your facial expression if the patient tells you awkward things such as homosexuality or mutliple sexual partners
17- do not joke with the patient at any time
