Hi Guys ,, I'm also awaiting for my results and I'm freaking out
I posted my experience earlier,

I got 212 in my step 1 exam , and that was to add to my stress before this exam

I studied for this exam for 6 weeks, 1st 4 weeks was around 3-4 hours and just practiced history taking with my skype partner , then live practicing all case (history taking, physical and PN typing) for 8 hours everyday for 2 weeks.

my main issue before the exam and during practice was always time management, I used to be very detailed in history taking , doing full PE so i didn't have time for closure and counseling.

So I decided that if I wanted to finish in time, I have to skip some questions in History taking, skip parts of physical examination and stay as focused as possible as long as it didn't affect your final diagnosis or establishing and supporting your DD.

Luckily, when I came out of the exam I was extremely satisfied with this portion , I didn't miss a single closure in my exam, I wrote 3 differentials with supporting evidences and most investigations.

but I did lots , lots of mistakes:

1- there were 2 patients in pain and lying over the bed, the first one I tried to make him sit down and he did, so i sat down and started history taking, the other seemed in severe pain , i asked him how can i make you more comfortable ? and then I sat down while he was lying over !!! I realized after 3 minutes that this was wrong so I stood up and told him that it's important to be keeping an eye on him ,,, the most sarcastic part I was trained about this topic in particular and still made the same mistake, also about the Draping, I can't recall when I draped both patients, usually i made sure that the drape was over the patient before PE, at all cases, but this lying down patient I can't recall if I draped him just before the exam or from the beginning of the encounter

2- I forgot to extend the foot rest in 2 abdominal cases , because I always concentrated to help the patient lying down and draping them adequately , till one the SP pointed that , I quickly apologized and did that.

3-I forgot to ask about illicit drug use in 3-4 cases where I thought it was irrelevant due to DD and patient age and group ,occupation in one, FMH in one, allergies in one .

4- I only used water to wash my hands , though I dried them well, this was because of being afraid to lose time

5-I always counseled about smoking, but there were 2 cases of alcoholic drinking, I think i missed alcohol counseling in one.

6- the patient in one encounter made a strange sound i though he was coughing, so i offered him a napkin, then he told me that something was stuck in his throat and he swallowed it I got blunted

7- my physical was super quick and super focused

8- PN, I ran out of time in the middle typing physical exam in 2-3 cases, I did perform the exams , but couldn't find time to type all of them in 2 cases
9- missed some points in typing of HPI also due to rush.

10- forgot rectal exam in 1 case- a couple of investigations, there were 2 cases although I established a DD, but I am not sure if I missed one or didn't get the right diagnosis, I missed one important investigation (EEG in a case )

11- one time patient was speaking then he stopped for a moment the I asked a question ,to find that he is saying another sentence, i stopped immediately,apologized and let him continue

GOOD things, always knocked , asked for preferred name, asked how to make him more comfortable, asked open ended qs followed by specific ones ,summarized, kept the patient well covered during PE, answered all challenging qs, made closure for all patients, counseled as much as I can, wrote 3 DD for every case with supporting evidences established most of investigations.

I am really hoping I can pass this exam, most of these mistakes are things that i have trained on, but when you get into that day you just get stressed about the time, and the only thing you could think of is to get to your closure before you hear the announcement to leave the room.

I really need your support guys and I wish you all the best with your exams and future

Default Things I DID/DIDNT do on CS

So my mistakes from what i remember:

1)Did know what the diagnosis was....so wrote some BS
2) Got so excited that i got the info that i needed from the hard patient that COMPLETELY forgot to ask Social history/Meds/FH/PMH and realized when walked out
3) Forgot to write in PN some thing that i did during PE
4) Forgot to ask about social history on one case which was relevant
5)Forgot to ask in some cases important questions for the diagnosis/history
6) Forgot to do MMSE in LOC

Things always did:
1) Always consult when needed (right away)
2) Summarized
3) Asked if they understand and agree with the plan or have Q's
4) Washed hands,draped,pull out leg rest , introduced myself.....etc
5) Always had enough time...which made me worried that i should have spend more time.


So those are Major mistakes that i remember!!!Im sure there were many many other.


What do u guys think???(especially about me missing entire section of Hx)???

hey just wondering if this will affect my score dramatically or not...i am really scared, so basically, in two of my cases, i delieverd my inital diagnosis during the pt encounter, and also summerized with some plans for those diagnosis, and after that, while i was typing notes, i realized that there's a better diagnosis (more supporting evidence) and then I made my summarized diagnosis(during the encoutre) to my second differential on my pt notes........
Is this gonna hurt my score a lot?? and i think i washed my hands in all cases, ask if any questions or concerns at the end.. that's the positive things i can think of during my exam, and also I dont know if i really show any empathy or not..that day went by fast....
lastly I pulled the leg rest with one hand and supporting pts legs with the other....(and this is of course after i washed my hands,) and I realized some people are saying whenever you touch pt's legs, u need to wash hands again? in this case will i be alright? or i will be getting marks off....
I appretiate any feedbacks from you or share of ur experience... thanks

I am waiting too! I did mine on August 5th.

Overall I think every body makes mistakes , no body can be perfect; after each encounter while I was typing PN I was remembering that I forgot to write this , that,... in the previous one !!!!

keep hoping that we all will pass.

Definitely freaking out about this score report.
Step 1 245, Ck 244. Step 2 CS failed first attempt. Will get second score Oct 8.

First time failed CIS. I get it. I was too focused on the ICE, and even though I said sorry to people when appropriate. I did not put any effort into the acting. And absolutely no small talk.

This time I knew better and practiced. Did some acting and chatted with most pts which they seemed to like, except one who called me on it for overdoing it.

However, 2 unexpected things occurred the 2nd time around.
1) I ran out of time while answering challenging ? in a case at the end. I actually spent too much time playing the CIS game. This led to me not getting to the investigations for that note. I mean I put 2 down before they called time, but definitely not adequate for the case.

2) My nerves got to me. Especially after running out of time on the one case (like the 4th one). I was definitely not on my 'A' game for the rest of the cases and made many of the regular mistakes people talk about. (missed some PE's, forgot to write NKDA in a lot of notes, probably could of done a better 2nd and 3rd DDx in a lot of cases, etc.)

What I am really worried about is the major mistake plus the small mistakes adding up and being a death sentence.

Anyone have experience with missing investigations?

No sweat, all minor. I have been doing a lot of reading and asking about cs mistakes and outcomes because of my own fears. The general consensus for what you have here is a resounding PASS.

I think your ICE portion is golden. I made similar mistakes the first time I took this test and was fine.

Your CIS however looks like it took a hit. But hard for me to gauge how bad. Plus, I am probably the last person to ask since I failed this portion my first time. But overall, if you got the impression that most your pts liked you when you left the room (excluding challenging pts who are difficult to read) then I would bet my money you passed.

Overall last year I made many mistake but now this time i am expecting some think great without any mistake. So leave past mistake and keep focus on future results..

Thanks CSared for your feedback, I'm praying that my errors wouldn't be too severe to cause me fail that test,

Do you remember your first exam experience in more details, how it went ? did you miss any closures/counselling ? did you ask pts about worries or concerns ?how was you physical and patient note ?

It is funny. My first exam I thought I nailed the CIS portion and was only worried about ICE. I did everything on the checklist. (ie. knocked, introduced, sat, eye contact, asked permission for PE, leg rest, closed/counseled, and said sorry a bunch when appropriate.) But the two things from CIS that I know I didn't do, because I did not realize it mattered until I got my results, was any acting or any small talk to win the pts over. I thought as long as I hit the checklist I would pass. That is not the case. You have to at least try to get the pts to like you.

I do not remember all my mistakes from ICE the first time. But I know it was a lot of the usual stuff. Missed PE's sticks out the most in my mind. I really only did heart, lungs, and pressed on the abdomen for all my pts. the first time. But that was good enough to pass. There were at least 3 cases where I missed an obvious PE, like the neck for hypothyroid, and it was fine.

Also, I did not review pts notes before my test last time. So just wrote an HPI. I did not put sections for PMH, Family, Social, etc. I only had the one paragraph with relevant info. And I actually tried to keep it short. I was coming off my EM rotations and this is how I was writing them there. Stupid of me to write the notes how I am supposed to in real life on the make believe test, but either way, still passed that part.

that's strange to fail CIS for that, I was taught that you have to establish rapport by questions like "how did that affect your life ?" or "what do you think that might be causing your illness ?""do you have any worries or concerns""do you agree with our plan of ttt" during washing hands , history talking or closure , but even that doesn't apply for all cases (ex. ER pts) , indeed some pts were mask faced especially the challenging ones, one thing I could remember, I was asked too many challenging Qs, not just one ..
it's worth mentioning that most of the time these faults you already know about it and trained on it, but when you are under the stress of the test and the strain of the timing it makes you make errors
Aghhhhh ! the wait for this result just drives you clueless and mental

Christ I hope I passed. Made so many mistakes I only realized after moving on to the next patient or stepping out of the room. GODDAMMIT. Getting my CK results on the same day as well, and that exam was just as bad. Christ.

I pray to god things go well, if not my step 1 score would be all for naught

hey,

and CSared did all cases in time with closure ,,, still CIS ????

Star hoping for result on OCT 8

hey there everyone,

i have a Q: i gave step2cs on SEPT. 3, do u think i'll get the result on oct 8? or it will be on the next wednesday ????

next Q: in my exam almost half the SP's were sort of blank faced, i was trying somewhat to show empathy and all. but ...

they were just too blank ,,, like no expressions ,,, sort of observatory face ...

but some sp's did smile at the end when i was leaving... i remember the young ones mostly ... the old SP's were very serious and ... turned off ...

i would like to know other colleagues experience... about SP's smiling or giving any expressions ....

Yes, I did complete all cases in time and I did close for all of them. But I did absolutely no acting or small talk. I did not brown nose the pts at all. I just went through the checklist. I thought as long as I introduced, eye contact, used gloves, asked permission, closed, asked for any ?s or concerns, etc. that they would have to pass me.

That is not true. I think it is more important how you do the checklist then actually doing all the things on it.

My opinion, it is extremely subjective. I was not rude by any stretch of the imagination. But at the same time, I just went through my routine with a rather flat affect.

But what do I know? I'm just one of the people that had to take the test twice, spend the extra money, and may have wasted 300k and 4 years for me and my family.....

You can check your release date on the nbme site. Pretty easy to find, otherwise I would do it for you.

I agree it is difficult. In real life I get along with my pts because I just respond in turn to their personalities. But if you take that approach with the sps it may not work out. Because a lot of them don't portray a lot of personality (ironically except the ones that are supposed to be difficult). They sort of just hang back and wait for you. I am like you and found it awkward and had it bite me in the butt. You just have to go in there and be the dr, but also be the one to initiate any small talk or fish for the sps to give you a little emotion to work with. Not easy.

I agree it is difficult. In real life I get along with my pts because I just respond in turn to their personalities. But if you take that approach with the sps it may not work out. Because a lot of them don't portray a lot of personality (ironically except the ones that are supposed to be difficult). They sort of just hang back and wait for you. I am like you and found it awkward and had it bite me in the butt. You just have to go in there and be the dr, but also be the one to initiate any small talk or fish for the sps to give you a little emotion to work with. Not easy.
On a side note, I actually thought about putting blunted or flat affect in a lot of notes the second time around to have a little fun with it. But decided against it. I know it is fine line. You are supposed to pretend like it is a real encounter in some ways, and supposed to know it is not a real encounter in others. Like when they say the pt is tachy but the pt clearly isn't.

Default very NERVOUS!

I have been hyperventilating every time i think about my cs result. I studied for about 4 weeks. My exam was on July 25th. I made the same errors as described by some members here. My first case, the pt. literally started talking even before i said hello. I could tell he was newly trained. However, i politely interrupted him and asked if there was anything i could do to make him comfortable and stuffs before proceeding. I was so nervous and managed to finish that encounter. My other cases went okay until my 6th case. Pt. complained of temporal headache and had a family history of SAH, so i thought i should do a MMNE on her. She was giving a clue to stop but i continued. This same patient was getting upset when i was trying to pull her drape up. I apologized and moved on. IT was so stupid of me. I didn't have time to counsel this patient. I was very upset when the announcement came on but managed not to show it. The patient gave me a "i told you so look" and shrugged her shoulders. i felt like crying at that point. After this encounter, i told myself that i really had to get it together or i was going to fail the test. All my other encounters went okay.
I had some cases that i struggled to finish the notes on time. and i had some that i finished ahead of time, i counseled all my patients except for the one mentioned above. Overall, all my patients smiled at me and so that got me comfortable.

What i did- I asked if there was anything i could do to make them more comfortable, i had a little chats in between, i washed my hands all the time, and always showed empathy.

for the CIS section, I would always start with my dd and workup and then work my way up. If for any reason i was running out of time, i would rush to finish my HPI which was always the last.

I don't know how i did, i am so nervous that i have failed this test, i am just praying to God at this point

You're going to be fine. It sounds like the first pt was one of the new sps that won't be graded. Only 10/12 are graded. 2 are so they can test new scenarios and new actors.

You are allowed to make mistakes. And you don't have to win over every sp to pass CIS.If you got good nonverbal feedback from the majority of your sps, then I think you're fine.

Thank you CSared for your response. I really do feel a lot better after reading your response. I really haven't been myself since the exam. I have been really depressed because i practiced well but found myself nervous in the earlier cases. I am hoping that i would at least pass. I would practice all day sometimes and some of the cases in FA i practiced over and over. Thank you for your kind words once again.

Guys my scheduling permit has disappeared. Is there any correlation of it with result as it was for step 1 and CK . Has anyone else's permit disappeared .

Took my CS on the 20th of August. Scared S**tless!
Done with my CK. Haven't taken step 1 yet.
I made a million tiny mistakes in almost every case but no major blunders.

Forgot to pull the foot rest out in one case.
Was wrapping up the counselling in another and the bell rang.
Missed the Social and Family Hx in one note but it was irrelevant.
Missed counselling the patient about HTN who had a 150/100 on the doorway.

Wrote two Dx in most cases. Three in a couple.
Established rapport with some patients and couldn't with a couple of difficult ones. Did as much acting as they did :P

I hope I pass!

Default My experience with CS

Hey guys, I took CS in May and just wanted to calm some of your nerves. I also felt scared/anxious for my results, at some points after the exam i realized my mistakes and thought i would fail. However, i did pass. I honestly don't think if you forget a few things here and there that they will fail you. It is the consistency with mistakes in each patient encounter i think that will cause a person to fail. Just stay positive and i hope all of you do well!

I appreciate your post. How about one major mistake plus the usual small mistakes for the other cases? My major mistake was running out of time while writing PN? Finished everything but only put one investigation. There were at least 3 other obvious investigations that needed to be done for the case.... I just didn't have time to write them.

Hey as far as i remember u cannot voluntarily choose a pm session. It only becomes available when am sessions are filled for a date. If u see a date with pm session available u can take that else u will b given am till its available. Do cross check with the information bulletin step 2 cs scheduling section. It is mentioned somewhere I remember having read so.

AAAAAAHHHHHHHHHHHHHHHHH!!!!!!!!!!!!!!!!!!!!!!

Definitely going to freak out all day....

Let's see, already failed once, nobody from my school that has ever failed twice has matched in the past.... Over 300k in debt.... Forced my wife and kids to sacrifice A LOT to get here.... I seriously promised them a better life but now we may actually end up worse off. Before med school we were poor, but at least we didn't have any debt. And all because if you stick me in a room and say my future is on the line, I become noticeably nervous.

But life is not fair, I knew that before I went to school... We will see what happens, GOOD LUCK EVERYONE!

Let's see, already failed once, nobody from my school that has ever failed twice has matched in the past.... Over 300k in debt.... Forced my wife and kids to sacrifice A LOT to get here.... I seriously promised them a better life but now we may actually end up worse off. Before med school we were poor, but at least we didn't have any debt. And all because if you stick me in a room and say my future is on the line, I become noticeably nervous.

But life is not fair, I knew that before I went to school... We will see what happens, GOOD LUCK EVERYONE!

Do you guys know of any trick to know if we are receiving results today (like scheduling permit disappearing for the other steps)?

my step 1 was released at 9:30 and CK at 10:30 EST... Anytime after 10 is possible. I would say unlikely in the afternoon unless they have a technical problem...
