Default these are the questions that haunt me.

1)do you have to do "recall" in MSE for headache, tremor, loss of consciousness or is it just done for Forgetfulness and confusion

2) do you have to do AAO X 3/ concentration for loss of consciousness?

3) do you have to do "gait" for headaches?

4) do you have to check for "concentration" in MSE for tremor?

1) for headache: depends, if it's migraine, cluster headache then, no, but if you suspect headache to be a symptom of something more serious (meningitis, encephalitis etc), then yes; same for tremor, for LOC you have to do it
2) yes
3) again depends on the case, for most cases I wouldn't do gait
4) if you suspect tremor to be due to Parkinson's, or essential tremor then no.
