Smile Got my result, I passed the CS

After a nervous time of long waiting, I got my CS results and thanks god I Passed. I did my exam in Houston 2 months ago.

I studied for 2-3 weeks FA, Neeraj notes and some USMLE World. I read some helpful online sites and blogs. Practiced mainly with myself (by imagining the sequence of questions and steps I am going to do) and few with family members. Most cases were from FA and the center staff were nice and helpful and the day passed smoothly.

My major advice is to practice a certain way of doing it in a systematic way, so during the exam u can go through without any blanks, as u cant stop and think whats next. Secondly be relaxed and deal with the patient as any regular patient you see in the hospital, be professional and show understanding to the patients complaints without showing any of your nervousness and maybe clumsiness.

Actually most SPs were cooperative and easy to deal with. Have to speak slowly and clearly, if u dont get a sentence or word from the patient best thing to do is to ask about it in another way and not by repeating the same question.

Manage the time wisely, and always leave time for a closure and summary put in your mind to always give whatever advice to the patient at the end, so even u forget counseling about smoking, alcohol etc,u still get counseling points.

Congrats!!!!..sounds great. So, what do you think which one is the toughest ? Step 1, CK or CS??..Maybe I should add the word "relatively"??.waiting for your reply.

Thank you! Actually CS was one of my life toughest exam experinces as you cant predict how you did in the exam and the lengthy waiting for the result when you cant expect what it might come up with, espically -like in my case - when a year of your life all is depending on one word pass or fail. But as an exam it is just like any other OSCE exam in the medical school with some extra touches you need to take care of.

Step 1 although it need much more lengthy prepartion but you can somewhat predict how you did and at least if you are passing or not. For CK i cant say as iam having my exam next month! Good Luck!

i want you to write your experience about Houston as i am intend to take my exam there

1- when is best to arrive to the city before the exam?
2- what is the daily expanse?
3- what about housing? and is it wise to stay before your exam anywhere and the day before the exam you stay in a hotel near the center?
4- what is the best flight to take to reach Houston?

you know this kinda question

I can tell my experience as it varies between one persons preference and another. I arrived there 3 days before the exam day. Stayed in a hotel near the exam center. It has free shuttle from the airport ( By taxi it costs around 25 dollars), they provide free bf and hot drinks. I think beside the hotel reservation 20 dollars can suffice u for the day unless ur staying for long time (hotel night around 55$ --65$ with taxes-- for usmle ppl). Any flight to IAH airport which is very close to the exam center ( ~ 15 mins away) is what you need, if your flight is more than 10 hours try to choose one-with a short stop in the middle of the flight. Congrats drmuh...i have some questions: 1-Q.in cs which part is more important examining the patient in a perfect sequence/or perfect xamination + perfect diagnosis? 2-Q.are the patients with gross/prominent findings given? (eg.pt with huge ascitis,severely ecteric or hugely enlarged organ that can be easily detected) 3-Q.Is it a great mistake if one misses any findings that's important but difficult to detect?

Congrats ! I have a query:

which is the best time to summarize the history? after taking the history or after completing the physical? i plan to present a brief summary of the CC before the physical, but worried if it will take time;or is it enough to just ask after history " do u want to add anything?"

I was doing it after the history taking and before proceding through the physical examination, I prefere that as it doesnt take more than 30 secs and finish a very important part and gets its marks. Unless u find urself critically late then u hv to do the physical fast and make a brief closure with at least mentioning the CC and its duration.

Another advice is the counseling, either make a list for it at the top corner and add what comes up during the history that needs counseling so u dont forget it, or counsel every issue directly at its time; even with one sentence -if u dont hv time- u get a mark for that. This safe u time as if u delayed it, first u may forget and second it take valuable time opening it again as saying "so u told me ur smoking etc" and bringing the issue to attention again. And then if u have time u can mention it again in the closure as I was saying, and please dont forget what we talked about cutting off smoking etc, so u remind the SP and make sure u get the mark.

A1: I think best is to do both, good sequence as u dont want to look ur not knowing what ur doing and messing things up, but what has to be perfect in particular is the examination of the related system or organ.
A2:I dont think u might face like these gross findings but it can happen though most likely not.
A3: Whats most important not to mess a simulated sign, for example a positive murphy's or positive straight leg raising test etc.

Thanks so much! These are extremely useful suggestions and I will try to implement them. You cleared my doubts as I am very keen to make a brief summary at the end of history taking, and ask the SP "Do you want to add anything?" Hope it works.
