Warning! CS for IMGs; Failed and then passed; My experience

I recently passed the CS after one failed attempt. The last time I had a problem with CIS part as most of IMGs do, actually my result report was so bad for CIS in the second attempt compared to other aspects. I was devestated when I failed the first time, and it will certainly affect my chances in the match, so I wouldn't like anyone to go through that experience and here is some advice that you might find handy:

- It is not easy at all. A lot of people think the CS is very much easier than step 1 and CK, this is so wrong especially for an IMG. It doesn't really matter how good you are in medicine in general, you have to be familiar with how they practice it in US before you get into the exam. Having double 99s certainly doesn't mean that you have a great chance in passing the CS, actually I find those who had good scores in step 1 and CK (as in my case) are more frequently failing the test than others. Neither is having a good spoken English or being able to communicate effectively with Americans is of great help in the exam. In my case I think the most difficult part is the CIS.
- Have a plan. CS exam is not a place to improvise for IMGs, I think and people tell me that I have a great spoken English, but during the exam words did not come by easily, and if they did come they were not always professional. The most important thing that helped me during my second attempt was having a frame to use with all patients, this will also help you not to forget about some forgettable aspect.
This is one example, but you can create your own frame to fill during the encounters:
- Mr..........? Hello Mr ..... , I'm doctor ....., I'm one of the hospital staff here, and I'd like to take care of you. So I need to ask you a few questions and perform a PE if that is alright with you. But before we start let me cover you with this drape to make you feel more comfortable. So how can I help you Mr .....? (listen to his complaint then say) I'm very sorry to hear about that, I will do the best I can to make you feel better as soon as possible.
- Continue with the HPI, and then say: OK, so now I need to ask a few questions about your health in the past (ask about the past history) then say: now I need to ask some questions about your health habits (aquire about the social history). Then say: Mr ... I would like to ask you some questions about your private life but please be assured that everything we say is in confidence as the rest of the encounter (and ask about sexual history then the family history)
- To sum up you have been having (sum up the complaint), is that correct? Do you have anything you want to add or anything we havn't discussed?
- Ok, so now we will move to the PE but before we start, I want to tell you that later I need to perform a rectal/pelvic/breast exam for you (I prefer to say this now because it is very forgetable, you only have to say it if the patient needs the exam)
- Mr ...... from the history and PE I think your symptoms might be caused by (and mention only 2 DD which are the most likely), or it might be due to something else, I'm not really sure right now, I first need to run a few lab tests and take some images of your body (explain if there is something specific) and then we will meet again and further discuss your case and its management. For now because I'm concerned about your health I have to advise you to........ (give advice about health habbits, and things related to illness), and don't forget to tell him that you will give him something for the pain if he is in pain.
- Find a tutor. find someone who not only have passed in the exam, but who has a very good experience in it, if you can't, a lot of ppl are recommending CSvidioes.com, I did not use it myself, but you have paid all of this money, it is a shame to fail the CS and put a black mark in your CV because you don't want to pay a few extra bucks.

This is really appreciated. At first I thought I could improvise (because I have good english and because I have done some customer service) I totally would be lacking words.

My exam is next week. Regardless of how it goes (hopefully great) I thank you in advance; I'm sure this will help out plenty.
