Thumbs Up Passed Cs with High performance in all three components

This was my second attempt, but I hit back hard and smart. I gave the exam in September in Philadelphia and got my results yesterday. I passed with high performance in all three components including my ICE. My CIS was the best amongst all. I kept staring at the report all day this was what I had longed for so long!
My first attempt was in philli too and I had failed in ICE with average performance in other two components. This failure was a serious blow to my confidence and belief in me. I tried my best to find out exactly what went wrong. Having failed cs I was even looked down upon other friends, Sad but also true. Shortly after I took a driving test to at least taste a bit of success and failed in that too. Why success is eluding me? I prepared again and passed the driving test after a week. It was a lesson for me-never get blown away by your failure and never to get overwhelmed with success! I was determined that for the second attempt for cs I will pass in the same dreaded philli, though many had advised not to take in that center.
Now what did I do differently? My second attempt was after I gave my Ck exam. That helped a bit with reaching a diagnosis. I prepared for 20 days.Most of the cases are very simple and First aid is more than enough.



ICE: To defeat the ICE portion which put a red mark on my application I worked a lot to improve it. Being an IMG I never did any observership or externship in USA. So had no idea how patient notes are written here. The patient notes in YouTube by karine dabrinyan were excellent. I went through all her patient notes and practiced typing in her format. I am not a good typer but practiced as much as I can in the same format as her over 200 times, all the cases in first aid .In the exam, for half of the cases I just wrote two diagnosis but supported it with at least 5 points in history. Physical examination supportive findings were mostly abnormal vital signs. Also I wrote everything the patient told, not missing anything in very short points. The Family Hx is very imp even if irrelevant in a case. Every patient had something to say. I used the copy paste option. Result: High performance in ICE.


CIS: It was my best performance. From very average performance (mostly to the left of borderline) before to way to the right. What did I do in 20 days? I did not have any online skype partner. With a baby it was difficult to schedule a time. I practiced with my baby’s doll. Set the cs exam timer in you tube, and practiced all the cases in first aid, including knocking the door, washing hands in nearby kitchen sink. I was a great entertainment for my 7 m old. I typed patient notes after every case. I recorded my every conversation and played back to modify any errors I made. Only the physical examination I practiced with my husband. On the day of exam, I did not get proper sleep the night before because of my baby, but I was determined not to get my tiredness from lack of sleep come in the way of my performance. Now the ball was in my court. I was wearing my best formal dress and shoes, Put minimal makeup; I imagined a picture of my family physician greeting me when I go to her clinic and then ya…..I did my best. This time I was not acting .I was the doctor going to take care of my patients. This was my clinic. I made them comfortable, addressed all their concerns. Physical examination only focused, did not do lung and heart in every case. Asked about their job, any stress in their work or at home, about studies if a student, about children if a mother..… At the end of the encounter asked if they agree with my diagnosis and treatment plan. I also asked them to call me any time if they come up with any concerns. I was not acting. I was just being myself. I forgot completely that they are going to score me. Result: High performance in CIS.


SEP: I am not a native English speaker. I did not do anything specifically to improve this because I can speak fluently. Result: One star touched the borderline.


In summary do not take this exam lightly. As they say, if you pass no one cares, if you fail everyone cares including your neighbor. Do not spend enormous amounts of money on the coaching classes if you cannot afford. I thank god, my husband who helped me to get a date by running a script, and usmle forums. I am finally ECFMG certified! Never get scared about cs exam center. The exam is standardized; go to philli if it is convenient for you. You are likely to perform better when you are less exhausted. Also never look down upon anyone who has low scores or attempts in any of the USMLE exams. Encourage them to move ahead with life. Everyone has a chance in this world. Thank you and Good luck.

Congratulations! Great that you persevered! How many hours did you study daily? Is FA enough?
I will certainly take it after I have done step 2 ck

Thank you! I did not have specific set of hours but mostly i was practicing whole day in those 20 days.Yes, First aid is enough.I bought kaplan core cases but personally did not like that.Minicases in First aid are also very important.Having prepared for Ck definitely helps but not very essential.
