Thumbs Down Failed ICE twice!

Hi, my name is Nickolas.

I am an experienced doctor and I think a good one too. I have been trying to get the ECFMG certification for about 2 years now in order to do a subspecialty in the US. I passed with the first try step 1 and 2ck with descent scores (236 and 212). BUT....
I failed twice the ICE part of the CS exams which I took both times in Philly. The worst thing is that I got high scores on CIS and ESP subcomponents and especially the second time I used all the ground rules (SIQORAQ DOCPAAA etc etc). And of course I did knock on the door, wash hands, support, did a closure etc.. Keep in mind that I have spent around 8 months in the US due to a scholarship I got from the university.
For my preparation I used FA and a lot of practice with friends and in a way at work with my real patients.

I honestly have no idea what went wrong. I got a really good feedback from most of the patients at the time. I am so frustrated and so surprised that I don't feel motivated anymore. The worst thing is that I do not know what to study anymore or how to prepare.

Please I know it is not your problem, but you are the only people that really understand the effort. Any ideas from people with the same experience would be appreciated. Thank u and god bless

I failed ice and CIS after taking Kaplan KAPLAN STEP2cs is waste of money and garbage
They give you stupid printout they did not cover what was on the exam
I definitely do not recommend Kaplan just use FA and study partners and you must practice more
Kaplan cs is definitely a total waste of money and scam

Thank you for bothering to answer my question. Did you take the online prep of kaplan with the live chat, videos etc?

did you follow the format of first aid for the pt note ? & where did you practice the physical examination from? ICE components are tricky

Thanks. Yes, I followed the format of first aid for the pt note. I practiced many cases. As for the physical examination, i studied the necessary way for cs from FA and practiced every day at the hospital and with friends. I am in internal medicine, so I know how to perform an exam in detail for most of the systems. In some cases ex. orthopaedics and neurology I studied from FA and watched videos for any extra manoeuvres

if u can post a sample of one of ur practice patient notes with everything included even what u would put for physical examination and DD and investigations.. i can help you figure out where u went if it was the pt notes, it is usually something very small that caused u to fail both times that u ever corrected bc u never thought about it ... otherwise ICE isn't much more.
ICE= physical examination, few imp questions on checklist and pt notes.
since u are practicing already and it is focused physical examination i don't think u messed up anything there. so it is probably your notes. bc physician notes diff than CS exam notes

Hey Nickolas,

Sorry to hear about it. I understand the frustration and the disappointment. Take some time to decompress, easier said than done, I know..

About your ICE, as of 2014 USMLE.org mentioned that they no longer have the patient history checklist. So the scoring is only based on your PE and PN. Most failed on the PN.

I agree that maybe we will be able to help you better if you post your practice note (not the recollection from your exam as it would cause you trouble with NBME). If you don't feel comfortable posting it, you can try to contact successful posters and ask them to review your PN. I did this too and the poster I contacted was very helpful.

You are doing the right thing to try to evaluate what went wrong so you can correct it next time. Take heart!

Hi Nikolas.I m sorry to hear this. I totally understand your pain.I failed in cis at Philly. I also did everything required. But I d like to suggest do not take Philly as your center and join some course like I am doing this goldusmlereview course in Houston. It is very frustrating indeed but take it as a challenge and you have to pass this time. I have heard Houston and Chicago are safer centers for img s. Best of luck.

Nikolas - man, I'm so sorry - it is really painful to read your experience:

Please - explain what went wrong in your Pt. notes:

1) have you used a lot of abbreviation or shortcuts - like for ABD: S/NT/ND/ BS+ / no HSM
2) did you used any symbols: like ↑, ↓, ♂, ♀, , F, C
