Default CS experience

i am writing this because I made alot of mistakes and still passed comfortably(high performance in all three) because I did the basic things right.
My mistakes:
-Couldn't diagnose 1 case at all.
-Missed a very pertinent/main ddx in another.
-Wrote an absolute bullshit ddx in another realised it when I pressed enter on the note.
-Got the order of ddx in another.
-In one case I did the PE over the gown(never do this)
What I did right:
-Was very confident and calm throughout the exam especially for an IMG who is still a student didn't freak out even in the case that I couldn't get the diagnoses at all.
-Managed the time really well in only one case the encounter ended when I was still in the room and that too I was about to say goodbye.
-Got the notes done and had time to review it. Didn't write any bullshit apart from that one case. In 3 cases I just wrote 1 ddx and didn't write 3 ddx in any case just 2. But my ddx were well supported so that debunks the more the ddx the better myth.
-Did pertinent PE in every case.
-Was very supportive of the patients listended to them and when they acted out of the ordinary tackled it as if it was a real patient and tried to calm them down.

3 Stars are pretty impressive, would you mind please if I ask you about material you used for your CS preparation :
1) FA CS - Tao Lee 44 cases
2) UW CS - over 70 cases
3) Kaplan Core Cases 3rd ed. Ph. Brottman - 43 cases
2) Kaplan CS Lecture notes 31 cases

cases from which of they recourse are most likely to appear in real Exam, you wrote in one case you couldn't figure out the actual diagnosis - was it something out of the book?

When and where did you take the CS - & after how many days you got results on your hand. Thanks much in advance for answering my questions,

Perhaps you wouldn't believe me but I hardly read the FA entirely I read the mini cases twice though and didn't use Uworld. I used to practice alot though with partners. No the case was not out of the scope of the exam I just couldn't get the information out of him as he wasn't too willing or was given instructions as such. I gave my exam in January.

Dr Drake please can u tell which center u took, and is there a difference in passing rates for IMG at different centers.

As FA CS book says accent is not a problem at all as long as you speak legitimate English...

I took it in Chicago. I don't think so there would be any difference between different centers this is a standardized exam. I prepared for about 30 days.Yes I typed after I practiced the case and I'd recomment everyone to do the same I didn't have any problem with the speed from the beginning but it helps you to make your note structure better. i think it was one of the reason that I got high performance in ICE despite so many f*** ups in ddx.

yu think I will pass with this:


I just took my CS few days ago, I made mistakes as everyone I think, but also as everyone I am sick worried about my result and it's gonna be painful to wait all this time until the results.

1st case:
I asked the patient to repeat to me the duration of cc,forgot to order pt/ptt for his DVT, and the notes was a little disorganized(positives then negatives then positives).

2nd case:
second differential wasn't medially accurate,PN a little disorganized as above.

3rd case:
pt asked me for HRT replacement medication, I just answered"advances in medicine are a lot and will find what fit you,lubricants are alternative for now" this was menopause case. second differential wasn't accurate or may be completely off!!

4th case:
asked the patient to repeat his medications names twice for me.didn't thank him after exam, but initially I said"may I examine you? oh thank you" I used this sentence in almost all cases but I forgot to emphasis on thanking almost all patients after PE.

5th case:
second differential was not good but close. didn't ask about sex. examination was not accurately described on PN.

6th case: case of fatigue, told me she sleeps 8hrs but not enough, forgot to ask about the sleeping pattern or interruptions. i show as much empathy as possible for her depression. asked me what's the reason for my weight gain, I said due to your depressed mood but we r here for you and will help with support groups and blah blah!!

7th case: forgot to talk about his diabetes follow up although the case was something not related to diabetes at all. I asked him to repeat his medications names for me(this happened for 3 patients).

8th case: I asked the patient to lie down, although he told me at first he can't , but when I asked him he said is this necessary, then I apologized profusely and said oh no of course not, I'm sorry I will examine you while your sitting up like that.

9th case: went ok.

10th case: went ok except that while i was asking the patient about ROS i said" ok let me see what I wanted to ask you!! oh I remembered do you have any chest pain?" that was dumb I confess it!

11th case: went ok,patient notes was a little disorganized.
12th case: went fine.

I was talking in confidence with all of them,
I didn't forget to put gloves on or remove them after PE,
I did closure on all of them,
I only ran out of time once and it was only when I was shaking hands,
I didn't shake hands for two patients one of them because of wrist pain and second was due to severe pain and not feeling ok at all,
I introduced myself well, i did transition phrases,
I did focused exam on all,
I wrote only focused exam on all PN, all patient notes were not as perfect as I used to do before going to the test, most of PN were disorganized but contained 90% of the patient encounter, I didn't ran out of time on PN,
i revised few of them,
I forgot to tell one patient about their coming rectal exam,
I forget alcohol and or sex habits in two patients.

It sounds - you performed very well, thanks much for sharing your experience. Would you mind if I ask you what type of resources you utilized during your preparation for CS - my exam on April, currently we did FA CS cases 2 with SP, menopause case is actually not given in FA CS, it is from UW CS - Thanks much, men. The best of luck !

I used first aid and some core cases. but i practiced a lot . first aid differentials or mini cases were my main goal, i did each one of them as long case with my partners.

We used to do that by studying the mini cases quickly, then we assign them to each other(my partner and I )and we practice them entirely as the rest of the long cases.
