Default When to counsel?

hi everyone,
just a quick question. when to counsel about smoking or drinking? during closure or as soon as patient say that he smoke or drinks while taking history.I hv heard different opinion on this. any answers will be appreciated.

Correct Answer

I think its a personal choice, I chose to do it as soon as patient is giving me red flags of smoking, drugs, alcohol or unprotected sex, I tend to forget at closure! and that can make you lose easy points.

I m practicing to do it as u hv mentioned.Another question if y can answer plz. What about typing notes in Capital letters ? Whats ur take on it?

well, one of my friends that have passed did everything in capital letters, so Im doing all capital letters too. its safer and easier.

thats what Im not sure,
Im typing the first impression of patient, VS WNL, HEENT relevant findings, throat and neck findings and then try to always include brief lung and heart and then the focused part, this is me, Im not sure tho.

I was wondering if when we ask the patient the OBGYN part, like last menstrual period, if regular, how many pads/tampons, I see that these are NOT included in sample patient notes either in FA or some Ive found online, does this mean that when they ARE NOT relevant you shouldnt include them?

its kind of confusing the physical exam patient note typing for me as well.

OB GYN - G1P0A0,LMP 4 WEEKS BACK,REGULAR CYCLE 28/4,LAST PAP SMEAR 10 MONTHs ago NL.l m not sure if this is right.

I was doing the same! G2P2A3, but I dont see written like that anywhere, thats why my concern.

I think we need to include the OBGyne history if it is related to the chief complaint and history. If not related, I guess we can skip it... Just my thoughts.
