USA Took my exam today in Los Angeles CSES center

Hello Everyone,
Today I gave a shot to take USMLE CS even if I failed my CIS portion in Kaplan mock Exam, we took on Thursday, I was so disappointed & frustrated – looked possible opening days in usmle.org for CS rescheduling – nothing was available till August … which means if I would cancel the Exam it is like crossing your all chance to apply for Residency in coming 2016 year Match

I enjoyed the class & people we had in Kaplan CS Course, Pasadena, CA – I don’t want use real names of people for confidentiality purposes as we mention each time before bombarding SP with hundred questions in our into – company was Great in the truly meaning of the word – it was great to get drunk with doctors from Poland, Philippines, Pakistan, Italy, Caribbean, Iran & Ukraine Med schools!

Last 2 nights I attentively went through our Kaplan Feedback 84 – pages in comparing with our ‘’purple text “book and all mini-cases with long 44 FA 5th Ed. Cases.

Always followed our Kaplan Into:

“HELLO, My name Dr. last name, I’ll be assisting you today. I will ask questions, perform PE, and share my thoughts. Confidentiality guaranteed. Please interrupt me at any time … bla – bla crap “

All patients were already draped – so remove that phrase from your into : “ I’ll drape you first” instead used another version we practiced Friday night with another doctor from American & Italian Med Schools “ I see you already draped. Is there anything I can do to make you more comfortable”?

HPI – portion – repeated back everything SP told me – so it covers Paraphrasing – at the end of HPI before passing to PAMHITSFOSS – summarized what I have heard – giving a summary was the another point I was missing on CIS portion of Kaplan Mock - hopefully worked .

When washing hands - each time asked Kaplan next 3 AAA – triple A questions to break the silence – how is that Affecting your life, what concerns your most, what do you think is going on = the truth is all SP answered to these questions as they were initially prepared – because otherwise normal person cannot make up this type answers instantly – so use that additional triple AAA questions! Especially, “what do you think what’s going on?” - will give you clue about 3rd DDX.

Closure – use all the time asked the phrase – do you understand what is going on, what we are going do next, are you agree my next plan of action – these were the points I was missing on CIS portion of Kaplan Mock Exam – so work on your mistakes!!!

Real Exam SP – well trained – will not steal your time like Kaplan SP – no Psychiatric Case at all – No God Damn Retarded time-eating Schizophrenic patients – all were nice, friendly I would say like the 78 yo. Male patient we had on Kaplan Mock Exam with Hip pain – recall that nice patient - thanks to real CS patients!

One of the SP was from Russia – I know my own people by our common accent – ask Doctor from Poland Med School – if you want to hear how it sounds (lol) - I was ready to speak her in Russian, because she was having trouble to understand my English – «в горле держал – типа Мария Ивановна не мучайте меня больше, мне надо еще вашу историю болезни за 10 мин накатать»

In general never lose your hope, and don’t let fear to paralyze your thought process – I never run the time, all the time gave closure - in 9 cases left with 1-2 minutes before “your Encounter is over, leave the room” – only in two cases made very weak 3rd diagnosis . Others were, I think all right – filled all Management Studies. In two cases put different order in one line – regular keyboards are much better then typing in Laptop Keyboards – you will definitely speed up!

Thank you for your post, it says really helpful. Was FA enough for all the cases you got?

Thanks for nice post.do u think SP understand different accents well?
In FA some cases are with lots of DD and i cannot finish in 15 min as for example knee pain,DM follow up?what about real exam,are the cases simple than FA?

Thant's right - handling in 15 seems pretty tough - that is why do focused exam - for DM follow-up, I only performed Fundoscopy and LE sensation, proprioception, LE DTRs - heart auscultation - it doesn't take more then 3-4 minutes.

I went to risk and used even up & down errors - male & female signs →

↑ - Alt+24
↓ - Alt+25
♂ - Alt+11
♀ - Alt +12

Started realizing - I have forgot to check for Vibratory sensation in DM pt. using tuning fork C-128

Oh, my God save our sole - And Bless MDs who is gonna check our PNs!!!

I had a question regarding management studies. While doing the FA practice cases I noticed the cases typically only needed 3-4 studies. From your experience filling them out completely each time, were the cases more complicated? or did Kaplan teach you to make sure you ordered everything?

Cases are not complicated at all - if you cover FA - long & mini cases pretty well back & forth, & in general you can 80-90% can finish PE too, 10% you will miss not because you didn't have time to do so - contrary because you couldn't realise what else should be checked - because of stress, time-pressure you just forgot it. Kaplan trains you as a hell - 1 min intro, 6 min all LIQOPRAAA & PAMHITSRFOSS, 4 min - PE, 2-3 min conclusion/closure/counseling. Usually 13-14 minutes enough if you're lucky with SP - some of them will forget their challenging question - start asking at the end of everything. It has happened at least twice with me.
