Wink Passed USMLE Step 2 CS exam (Dec 7, 2011)

First off, here is the time break down.

15 minutes = Data Gathering
10 minutes = Patient note
You do 12 patients total.

You will have a lunch break (30 min)
You will have a little break (15 min)

Going into my exam:
I had completed 40 weeks of clinicals

How I studied:
Studied 3 - 4 weeks for about 1 -2 hours a day
Only used First Aid for CS
Went over the cases 3 times
- 1st time was by myself to get my questions down
- 2nd time I used a study partner
- 3rd time I used the same study partner
Studying with a partner is key.

My experience:

I was really nervous with my first encounter. I had ran out of time with my first encounter. It felt that I just got into the room and I was being asked to leave. Never got to do counseling. Typing I was so nervous my fingers weren’t working well. Fingers shaking. I forgot to put a couple things in my note

After that, I made some "rushed" mistakes:

I forgot a lot of questions related to the problem they were presenting with. I sometimes forgot to post fundoscopic records even though I did it. I forgot to put allergies for 1. Forgot some differential diagnosis. Forgot lots of questions that I thought of afterwards.

Draped the patients every time I walked in.

Basically you are not going to be able to do all the examinations you want to do. There is just not enough time. Make sure you do Chest and Abdominal. Everything else would be just gravy. Make sure you ask if you can take off the gown to do your exam and make sure you tie it back up.

I washed my hands when I was about to do a physical exam. Counseled while I did it about smoking and things along those lines.

At the end, I did the closure. Repeated back what she said the problem was. Told her that I needed to do some tests before we can pin point whats wrong. Asked her if she had any questions. Then I said we'd be in touch.

I thought I failed walking out. After a while I thought I did OK. Enough to pass. We’ll see, I should get it back Dec 7th.


Update: PASSED with HIGH Performance, all stars hitting the far right.


My notes I did a little paragraph for the HPI, but then switched it too bullet point. Example:


Joe Doe is a 24 y/o AA male, who presents today with a hx for farts. Farts started after eating Mexican food and has progressed into a burning anus syndrome. ........

ROS: Negative, except noted above
General: WN/WD M/F NAD
Head: Normocephalic
Chest: RRR, nl S1/S2, No M/R/G
Abd: soft, non-tender, non-distended, +B.S.
Neuro: CN II-XII intact, .....
.....

you were working just on FA?
Did you have stations not covered by FA?
is counsulling station new?
From where we can prepare prepare for counselling cases?

congratulations...what do u think what were the key points tht u made sure u must cover as i assume washing hands..dealing good with the pt...counselling n the way u closed with the pt were the things u focused more...n please show us a complete pt note sample the way u did in exam..

Did the exam in Atlanta
ONLY used First Aid Step 2 CS, just the cases in the back 41... no mini cases
Never looked at any video, seems like a waste

When I asked questions, I first did head to toe questions. Then I told them if they answered yes to anything we would go more into it.

FA is a learning resourse. It doesn't matter what cases you have, FA teaches you all the questions you need through repeating questions and getting a system down in the 41 cases. So it doesn't matter what case comes up you'll be ready.





My Bullet points

ROS: -'ve except above
General: WD (Well developed), WN (Well nourished), M or F in NAD (no acute distress), mild/mood/major distress
Skin: No rash or eccymosis
Head: Normocephalic, no signs of trauma
Ears: EAC patent (ears are clear), TM (tympanic membrane) intact, no fluid noted
Eyes: -Sclera and conjunctiva clear
-PERRLA= Pupils equal, round, react to light, accommodation
-EOMI= extra ocular muscles intact
Nose: Non-inflamed, Tonsils present/abscent, no exudates
Neck: -FROM= full range of motion
-Normal curvature
-No JVD (jugular vein distension)
Chest: Clear to auscultation and percussion
CVS: -RRR= regular rate and rhythm
-RRR with S-1, S-2, no G (Gallop=rate >100bpm), R(rub=friction), or M(murmur)
Abdomen: -Soft, non-tender, BS +/- (Bowel sounds)
-HSM (hepatosplenomegaly): Y/N, Surgical Scars Y/N
EXT: -No C (clubbing), C (cyanosis), or E (edema)
-DTR (deep tendon reflex)= 2+
-PP= 2+
G/U: Male: Normal external male, no lesions or urethral discharge, bilateral testes, no masses
Female: Normal external female, no lesions or vaginal discharge noted
Back/Spine: FROM (Full range of motion), normal curvature, no spasms or tenderness noted
Rectal: Good tone, no masses noted
Stool: Heme +/- / differed
Neuro Alert, O (orientation) X 4, CN II-XII intact motor, 4-5/5 in all groups
Sensory intact to LT (light touch), PP (pin prick) and proprioception

I have FA CS 2009. is it old?

I bought the book a month before hand. Never seen the 2009 book. If it has 41 cases in it, then it would be the same
