Hammer and Otoscope Very painful joint exam; what to write in PN?

if we dont even touch the painful joint nor move it, wat are we gonna be writing in the pt note?

You can say something like this:
"Joint range of movement was extremely limited and appropriate joint exam was not feasible as it was so painful to the degree that patient was unable to move it at all and did not allow even palpation"
