Default CS Exam Experience

Mistakes:
1) Did not mention Vital signs on PN. Just mentioned- pt tachycardic/ dyspnoeic. Ignored altogether when normal.
2)Forgot Social history for 1 case
3)Missed a 1 important Physical exam in 2 cases
4)Forgot to shake hands
5)Forgot to ask social hx in one case
6)Wrote down a vague diagnosis for an obvious case
7) forgot vaccination HX
8)few had only 2 DDx
9)as a handful of physcial exams where normal- I left PE blank when justifying the diagnosis.
10) Did not note down on PN a few things that I asked the SP

Things that went right:
1)Summarised for all but one pt.
2)Closure for all
3)counselled all (i think)
4)Introduced myself, Did not forget pts name
5) answered questions the pts had
6) helped them lay down and sit up, draped them, helped them with the gown after exam
7) My SPs had no problem understanding what i said or asked.
8)Confident about diagnosis in 10 cases
9)finished before time during clinical encounter, never rushed the pt

More wrong than right? This is driving me crazy! Any input would be great!

Thanks Dheera. Just remembered that I did not ask vaccination, birth and development hx for my peds case. So worried.
