Question How we gain marks for patient notes in CS exam

Actually I am confuse and i need some help I want to know what way exactly we gain marks for pt notes.I mean i checked usmle website that state that we gain marks on all the information we provide about pt positive and negative but they havent mention anything about grammar,spelling mistakes, punctuations,caps,small etc . I mean while typing its very hard for me to keep my thought related to this all and even remember what was the history and above all there is kind of my grammar is very weak to tell you frankly so need some advice.Typing is already sucha headache and this all things too so kindly someone reply pls...............

tips for patient notes:
1. list only the history and signs which you have actually gathered/done
2. dont provide any false information
3. avoid spelling mistakes
4. differentials should be listed according to priority
4. investigations: from simple to more complex

grammar and punctuations is that fine to have few mistakes infact one of person told me its allowed and i do trust him just thought to ask you also.

False information i kn is wrong but I am wondering if someone really miss to see some finding then what

One odd mistake is allowed but you should not make too many of them. Now that the exam is scored, you should be more careful. My senior suggested this DVD to me "Jaypee's Video Atlas of USMLE Step 2 CS Cases & Tips" and i found it to be really useful. This DVD along with FA allowed to me ace my exam in the first attempt.
