Question Why rectal exam for Erectile Dysfunction?

When a man presents with erectile dysfunction, what is the main purpose of including rectal exam in the work up? Could someone please explain if they know the answer.

Who said you need it?

You only need to do the cremastric reflex to verify nerve function, you don't need to do rectal exam for the purpose of ED alone.

FA says you should mention to the patient that you would like to do a dre later. they mention cancer as one of the causes of ED so i am guessing that CA prostate or rectum that is so far advanced and infiltrates the nervous plexus can cause ed, i am not sure though.
please dont do a cremasteric reflex, dont go near an sp's groin for any reason.

you have to do a rectal exam in order to feel for the prostate. I would assume that if there is cancer of the prostate. Metastatic disease can infiltrate the bones and potentially the nerves that supply the ability to have an erection. Genital exam should be done if the patient is suspected to have hypogonadism because of decreased libido or people who do not have noctural or morning erections.
