Default Few questions about CS

I have a few questions regarding the exam:

- For the ROS, would you go through all the points quickly and if it is positive ask more questions for that specific problem (for example Is there any change in your bowel movement? If yes, i would ask about color, blood in stool etc) or would you only ask the questions which you think maybe help to confirm the diagnosis?

- People say, they wrote down 3 DD for the complaint before they entered the room. But for example for chest pain in a 46y old male, there are so many DD, how can I choose 3 DD before I ask the questions? I was thinking of studying the DD for each complaint from the long cases, and after the HPI I would know, which 3 are likelly. And then I could adjust my remaining history and physical to confirm this 3 diagnosis. Are there any other suggestions?

- If I decided which DD are likely, do I write the work-up to confirm the diagnosis or also to exclude others, which are not in my list of DD. For example I have MI, cocaine use and GERD for chest pain, do I still do a CT cest to exclude PE?

1. for ROS- just focus on yes or no answers.... if yes then go into details because many times when you cannot find a 2nd or 3rd D/D it helps to ask ROS and figure something from there but otherwise quickly move to the next system if nothing is wrong.

2. i wouldnt waste time on writing D/D before entering the room i would rather spend that time sitting at the computer and typing it up. when you begin your encounter and history. 2 D/D will usually pop at u from his presenting illness and 3rd Differential is little harder.

3.confirm first and exclude.
