Question Rewriting the doorway vitals!

In case of LOC can we write normal orthostatic vital sings in the PN without rechecking the BP pulse and stuff??

Write the vitals that are given to you in the patient note. Even though you are told that you can recheck, you probably won't have the time to recheck the BP and pulse and the general rule is even if you do, whatever vitals that were given to you in the doorway is taken as the correct value.

Yes you can write the vitals mentioned in the doorway information. But in a case with LOC try to measure the BP.
