Question How to organize the blank paper for taking notes?

How should we organize the blank paper (scrap sheet) that they provide for history taking notes?

Write patient's last name in the corner, that way you will remember what to call him/her by. Then write all your vital signs and that you can directly transcribe this on your PN. Write a few d/d that come to mind associated with the complaint. Finally, write the mnemonic that you will use for the patient encounter, so you can use it as a checklist/make notes
