Unhappy Another "i think i failed CS" post

.... i think i failed cs.

took it yesterday in Chicago, as my first attempt... i was a complete WRECK for the first half!

first case, i was frazzled and couldn't get a good DDX. in the middle of my physical the time ran out...just got through CV and resp, only stated neuro and didn't even get half way through and neuro was so important in that case. I remember a loooong, awkward pause in the middle of that case where i literally just stared at my paper and couldn't think of what to ask next... exactly what I was fearing all along. time ran out on my note too

Second case was better, could have used more time on my note (didn't get to back up my DDx with Hx and physical exam findings)

another case totally eff'd up the challenging question. bombed it. it was BAAAAD. i have no idea how i messed it up so badly. I feel like a idiot. I can't believe it, I knew what i had said was the wrong answer the moment I walked out of there but also knew i couldn't go back in. It was a weird "peds" case which i honestly just treated like an adult since the kid had permission.

Fourth onwards was a blur but pretty basic stuff, most of those i was able to do CV/Resp and a focused exam and leave the room early... but for some reason i (and apparently a lot of people) felt like there must have been more to the cases... that it couldn't have been that simple

I had a mild-difficult time with DDx for about 40% of the test because they were so vague that it was hard to really rule anything out -- even with my very thorough ROS questions.

What I did wrong:

- messed up the challenging question for sure in one case
- 2 partially incomplete notes (ie. missing ddx backup)
- 2 tough neuro/psych cases which really tripped me up... i did much better and was super composed on the last one but still wasn't certain of the diagnosis
- forgot to ask social history in 1 case
- 5 cases were really tricky IMO
- I could only get through a complete neuro exam in 1 case (including DTR, CN2-12, sensation, strength and whatever signs i needed to test)
- most of my PE was negative with the exception of a MSK case... one patient I thought was breathing shallowly on purpose so i put "decreased breath sounds, shallow breathing" as my supporting PE findings for a couple ddx's... don't know if he intended to be breathing so shallowly



What I (think) I did right:

- stayed positive, tried to be super friendly inside and outside, ie "stayed in character" haha pretty sure I annoyed some people but whatever...
- great intro, counseled as the interview went on as to not forget anything and make sure i could rack up points had i not enough time to close
- congratulated/applauded on smoking cessation, getting over a serious medical condition etc
- summarized after taking HPI ROS, asked if there was anything else they'd like to add or ask me
- closed well, gave my impression and said that the differential isn't for certain until the labs etc are back blah blah meds after results blah
- asked if they were comfortable in the room before starting hpi
- didn't repeat painful maneuvers
- helped pts lay back, sit up, pulled out the leg rest
- respectful draping
- asked if the SP would like some water or a tissue after every cough, and for every thyroid exam
- explained what i was doing and asked politely to move the drape during the PE
- smiled, smiled, smiled, eye contact, personable, small talk like about their occupation
- replaced "um" with "well," lol
- wrote a decent H&P albeit jumbled for a majority of the cases



Anyways.. i was just wondering if anyone has ever had this experience too and passed ... ugh.. only slept 3 hours before the test and I can't even sleep or eat now. this anxiety sucks!

Took it last week and was in the same state of mind the day after my test. Screwed up a lot, missed some closing and counseling because of time issues, same for the note where I ran out of time for some of them or bc of time constraint missed to write some important informations that I knew from the Hx for the case....Put a world for another one which mean entirely different thing, despite my reasoning in this case was good...
Well, the only thing I can tell you is that after one week the pain is not so bad anymore and I sleep better. Try to go back to your life, see friends, do other things meaningful for your studies or whatever. The time constraint in this test is definitely a major issue, I know most of my mistakes were because of this so short time frame. This is the game, we have to go with their rules...Let's pray for a great outcome for both of us...
I saw the next period is after the 13th of April... I wonder if we should expect our results around this time? They say most of results are sent the first Wednesday of the next period... The sooner the better.

Thanks for replying! IT'S SO TOUGH OMG i can't think straight. trying to study for CK is so difficult when it's nothing but constant reminders/mini panic attacks of "omg did i put this test" or "omg this should have been a DDX, did i put that down" or "omg i totally messed up those two challenge questions ARGH"... I'll never forgive myself for some of the stupid mistakes made (unless i pass, then i'll live and learn hehe).. I had a hard time backing up my DDX since most of my physical exam was negative... ugh. but hey, we're in this together

This standardized patient experience was really strange, I'm not use to fake signs, so the few I had felt totally weird and different of real ones.... Oh well, better not to think about it now. I took one week break to feel out of the fog post exam, and now back to study (step 3). Whatever is done can't be changed, so, better to keep our brain busy with something else I guess...
Good luck for step 2CK, and hope April 13th will bring good news!

I took mine a month ago in Atlanta 12th feb
Most of It is now a blur but every once In a while I get these real chills when I remember things I messed up !
So first I'll tell you what I feel I did wrong
-a case where there were 2 diagnosis in order which do fit perfectly (I just got the first one and missed the second one ) though the diagnosis I wrote instead isn't totally wrong /messed up so that's one concern
-there was this one case where I've written a diagnosis which as (ligament injury of a body part) Coz all the findings were negative ! I know I should've been more specific
-i had this one case where the patient wouldn't say yes to anything ! In fact she contradicted my statement on my face
-might have missed counseling totally in 1/2 cases

Things I feel I did right
-always knocked
-always smiled
-was emphathetic
-washed hands
-focused accurate examinations
-always asked /informed patient of was going on

My main concern is! Does missing a couple of diagnosis in different cases (not all 3) mess up the chances of passing the test greatly ?

yeah same here, it was tricky. I'm trying to pull myself out of it, luckily i feel a little less bothered today than yesterday. Study hard!! Kick some ass! and thank you hun

I heard that so long as you're in the ballpark of the diagnoses you're good. It's apparently not so much about if you hit the nail on the head -- more if you know generally where the nail is and how to use a hammer haha

this is the worst. i'm absolutely convinced that I failed. I can't believe i answered a very straight-forward challenge question wrong -- one that i never would have in real life! It's eating me up. I hope the SP's grade kindly lolol

I understand how you feel ! Coz I feel the same way ! I have 2 kind of moments in the same day !
One when I panic and convince myself that I failed ! Then after a while I'm like wait that's stupid , I haven't really messed up that bad to flunk the test :/ ! Frustrating especially because I can't focus on CK

yeah that's what mine says.. although i wonder if that is just because the scheduling period is over? who knows.. ugh.. i wanna throw up. Anxiety was pretty under control for the past couple weeks until now

Hey I'm in the same boat you are! Took it a week ago, and I'm also having sleepless nights thinking about all the stuff I missed, especially on the PE. Some PE maneuvers I missed were really important and could've led me to the right DDx. But because I forgot to do some of them, I feel like I will fail ICE!....That happened I believe in 4 cases. But I did remember to close out every case, finish my note in time, and document whatever I could, and came up with reasonable diagnoses for the most part based on what I had. But still feel like everyone else did so much better than me and that I'm the only one that thought the test pretty difficult!

Hey GTfan404!
Honestly, this test is a weird one. There's no way of knowing if you passed/failed because i think each test is curved differently. I don't thikn you have much to worry about because they don't expect you to hit every single sign. I'm pretty sure i forgot to test something important in every case haha so we will see how I fair today. After talking to my friends they said i had a very difficult test since a good portion of my cases weren't in FA. I hope they curve it generously!

CONGRATS EVERYONE!!!

so glad we never have to think of it again!! Thank God for that! and Thank you for all your support, too. It definitely helped keep me sane for that month there haha!

If there's anyone reading this who doubts they passed -- think again. I made some TERRIBLE and SERIOUSLY BIG mistakes but still managed to pass!

Yep ! I'd like to share it too ! Now I was paranoid about missing differential diagnosis ! They don't affect it that much ! My ice was totally on higher performance (no border lines ) so was CIS . SEP I got a star (yay) . So bottom line being this exam is a stupid stupid stupid stupid brainless anxiety provoking exam which accomplishes nothing. :/ Goodluck to everybody and thank you for the support when it was really necessary .

Congrats to everyone.

After many panic attacks, sleepless nights, uncontrollable bouts of cry and serious thoughts of giving up on becoming a doctor, I finally got my result. Thank God I passed. I wish the best of luck to everyone preparing for the exam.
