Question Shaking Hands

During the patient encounter when the patient offers to shake hands with us on entry, and we just been with another one, should be still shake hands with him/her? Can we say "I am sorry, you are a very decent person but I am more caring, I just been with another patient and would like to wash my hands before any contact "
