Default Another Step 2 CS freakout

Ok, so I am in IMG, took my CS a couple of days ago, and I am seriously worried I failed the exam!...The mistakes are endless, especially in the PE.

Case 1: Went ok, except forgot to do MMSE when it was def required. It could've helped me support a DDx that I put down.

Case 2: Went ok, except after I finished counseling and was about to end it, I realized I forgot a key PE maneuver. I managed to do it and I'm glad I did, but I'm not sure if it will cost me points.

Case 3: Went very good

Case 4: Went very good

Case 5: Didn't really know what the DDx was, but I think I did all the PE maneuvers. But the pt. seemed very annoyed with me the whole time. Also, did one PE maneuver at the very end after closing and summarizing. Only then did I remember it needed to be done.

Case 6: Horrible MSK case....I had no idea what the pt. had. I realized afterwards I forgot some key PE maneuvers

Case 7: Went good.

Case 8: Went good

Case 9: Went ok. Except once again, I forgot a key PE maneuver that could've been used to support or refute a DDx I had written

Case 10: Had no idea what the Pt. had. So put down 2 reasonable DDx but realized later that I forgot to check peripheral pulses when it was def warranted.

Case 11: Went good

Case 12: Went ok. Forgot to do a key PE maneuver that would've helped me support or refute a differential I had.

My worry is by forgetting some key PE maneuvers, this will also impact my note, because I will not have those PE findings to support my DDx that I had written. I feel like some of the PE maneuvers I missed could've been positive! I'm afraid I will fail ICE because of this.

What I did do right:
Closed every case. Responded to every emotional concern that the patient had. Counseled every case. I feel like I good history out of every patient. Overall, I feel this part of the exam I did well on. It's the ICE that I'm worried about because of the PE!!!! Any feedback would be appreciated!!

The physical exam is just one aspect of this test. I wish I could tell you that you pass but there is no way to know exactly their grading system... Everybody feel pretty terrible and depress after this test, and so the best thing to do is to breathe ( yeah, I know, easier said than done), and try to think to something else until you get your report... If reading the mistakes others did and still pass may help you in getting more relax, you may go over this forum and find all the posts. I think somebody posted a shortcut recently on a " I think I failed step 2cs". And remember, statistics are overall in your favor as there is more success than fail

Yeah it's just their mistakes don't seem nearly as major as mine. Like I missed key physical exam findings that could've helped me support some diagnoses or also make some diagnoses less likely and listed lower in the note. But I'm afraid because I missed those, I'm really screwed. And in general, I didn't spend a lot of time doing a thorough PE. Just did it on whatever system was in the CC and what I thought was relevant. I never did a full neuro exam on anybody!! Just did bits and pieces of the neuro exam.

A good neuro PE can't be sqeezed in this test, we have to take shortcut. And honestly the fake findings can be puzzling. I don't know if you read Phloston advices on student Doctor network but he was saying that a good closing is better than trying to find something on PEnwhich is fake anyway... He passed. I did my share of mistakes and I'm beating myself up for that, and I still have until Wednesday to know if that gonna cost me this test... But anyway, what you did is done, and I'm sure you did your best! This test is tough, but now try to do something else until your results, because torturing you about what you could have done better will not help in any way. Obviously in medicine, they apply the good words " what doesn't kill you make you stronger"... I bet we are super strong :stuck_out_tongue_winking_eye: . Let's keep a high spirit!!! And there is a life beside USMLE, thanks God for that :blush:

In how many cases did you mess up the differentials? I see on the forums most ppl messed up on only 1 or 2 at most...For me, in two cases I had no idea what the patient had. In another one I had some idea but forgot key PE maneuvers. In another, I forgot a key PE maneuver and as a result, listed the most probable diagnosis 2nd on the list. And finally, in a fourth screw up, I forgot to do MMSE when it was def necessary. I sort of knew what DDx to put down, but don't know if they were in the right order. I feel I'm a total scrub, especially at the PE. This is really killing me!!! Most ppl talk about how they messed up on trivial things, but mine are pretty major!!

Honestly the accurate memory of all my cases has already began to fade, but I remember one case in particular where I forget one very important DDx, and I'm actally not sure the one I put was the accurate one, or the good one was the one I remembered too late...(but some people told me there is no really a "good one", they are testing our reasoning and capacity to think of differentials fast). One time like you I was already in the closing and I still had the gloves, and checked one more thing for the physical . It was awkward, but a necessity for the case. I screwed up the interview with the first patient, as I didn't had time for closing, and in one case I screwed up the patient note, because of time I didn't had time to fill one part. In 2 or 3 cases I know I forgot in my patient note some important information I had from the interview with the patient but I just forgot to put them because of trying to type fast (I'm an average typer), and it was important to prove I was right in the order for my differentials. One time I confused one English word and it means a totally different thing, though my reasoning was good. My results in ICE and CIS are nowhere touching the borderline, but that probably what makes me touch the borderline grey area for the English part... I still passed. I was so depressed after the test for these stupid mistakes which I know I made essentially because of the stress and time limitation... So, believe! I know it hurts. But hopefully, your medical reasoning, your empathy and good closure, your patient note as much detailed as you could gonna make it! Keep us updated please!!!

Congrats on passing!! I wish you the best! BTW, in the examination rooms, did you notice that there were two tuning forks available or just one? I might have used the wrong one, and that's also killing me, because I couldn't get anything out of the hearing tests!

I remember seeing 2. But I'm pretty sure that since it's not a real patient, you just need to fake using it, if you had to, whichever one you chose, it should have made the trick if there was something to find...Because the SP gonna give you the sign if there was one, even if you didn't made it vibrate properly, it's ok
Courage, try to not overthink too much to the test now, it's over, and hopefully for good!!! Wishing you success too!
