Help Smoking and Driniking Threshold for Counseling

Can anybody tell me at how many packs a day of cigarette n at how many drinks a day for alcohol we should counsel the patient?

And whats the most appropriate time of counseling, when patient gives his personal history about smoking and alcohol or by the end of encounter with the closure part of interview?

There's no absolute number. Use the CAGE questionnaire, if any question positive then it's a good idea to counsel. This issue has been discussed here When to consider drinking signficant!

You can do the counseling immediately at personal history or later on, it does not make a difference, just make sure you don't forget to do it. This issue has been discussed here
Counseling during history taking not waiting to the end of the encounter
