Default capital letters during PN

Hi all:

When we write a patient note, does it matter whether i use capital letter or not?

For example:

physical examination:

neuro: Mental status: Alert and oriented x 3. Cranial nerves 2~12 grossly intact. ( This is from FA)

if i write:

neuro: mental status: alert and orientedx 3; cranial nerves 2~12 grossly intact; motor: right hand resting tremor


is it ok to write like this? because for new PN there is no so much time to keep changing to capital letters.

I, honestly, do not think that it matter how you write it. But when you capitalize things as you should, it looks more professional and may be easier to read.

Definitely practice on your typing speed. Make sure that you are able to finish it in less than 10 minutes.
