Listening any opinions for my PN?

Hi all: for the new PN, i try to write the important things in my history. Here is what i wrote in the dizziness case, please give any opinions as long as u can.

HPI: 53 yo m c/o dizziness for 2 days. Feels dizzy when he gets up from the bed or lie down to sleep. Has left hearing loss. No tinnitus, trauma, affects his daily life. Has N and V, and has diarrhea for 3 days. No chest pain, palpitation, SOB, abd pain or joint pain. No weight or diet changes.
ROS: negative except as above
Medication: Furosemide and captopril
PMH: HTN for 7 years
PSH: appendectomy when young
SH: no smoking, EtOH 2~3 beers/week (CAGE: 0/4), works in an insurane company, sexaully active with wife.
FH: noncontributory

I don't think N and V is standard abbreviation for nausea and vomiting. I think we have to use the ones in the usmle CS booklet. What are your thoughts on this

Looks really good. Only one thing I would say is that I did not understand where you wrote "affects daily life". If it does affect his daily life then write it as a separate sentence if it Does Not then you need to add the word does not affect daily life. Otherwise very nice!
You could shorten the SH to save time and instead of work sin an insurance company put Insurance Agent, or if he is not an agent then write Secretary, Manager or whatever his position is! Good Luck!!
