Default weight loss and loss of appetite

Hi All,
I am watching the new cse videos, now on the section of physical exam findings of like a differential dx of lung cancer, they write weight loss, decreased appetite under physical exam findings. Is this ok to write in the real exam? To me this seems like hx findings since the patient is the one who tells the doctor/examinee that he /she has decreased appetite and has lost weight .Please advice.
