Wink Step 2 CS New Pattern TIPS, How to PASS

hi all,
first of all I am thankful to God and all the people who in any way helped me PASS this insane test...
During all these two months of waiting my tension was relieved only because of the forums written by people especially the form ' post cs feeling' was very helpful...so I also made up my mind to right one to help and guide others...
Well i be very concise as i have to prepare for step 1
1- cs is changed now but the only part really changed is the patient note portion you have to speed up your typing speed...
2- there is no book on the new pattern so its not very clear what is the best way to right the pertinent positive and negative findings to support each of the diagnosis ,,,what i did?? i wrote full length sentences like' smoking history for the last 30 yrs or shortness of breath etc...my friend also did the same way and he also passed the exam
3- the 2nd portion that they say is changed is CIS..but i dont think there is much difference...i and my friend just followed the old ways of passing CIS and we both got high performances...THE KEY IS TO BUILD RAPPORT,SLOW SPEECH,COMPASSION,....EVEN IF YOU HAVE TO BE VERY VERY VERY FOCUSED ON PHYSICAL EXAM(TRUST ME ON THIS)

........................My exam experience and mistakes:
1- case: went good,,,i built good relation,,the lady also very nice,,however when i asked what meds she has been using she named some drugs i never heard of and i was like haaaaaaaaaaaan...but this is where the patient will assess you...if yo dont know the med ..dont be over clever just do WHAT YOU WOULD DO IN UR REAL LIFE ENCOUNTER,...and i did just that...i got hold of my self and asked the lady can u spell the meds for me....she smiled and said i dont know either doc ,,i replied its ok i will try and find out what is the generic name of this med...thats it........i did fumble a lot...especially in my speech...and many times during my exam i could nt understand what the pt said...THE KEY IS NOT TO TELL THE PATIENT TO REPEAT WHAT HE/SHE SAID BUT TRY AND SUMMARIZE WHAT YOU UNDERSTOOD(BE IT WRONG) AND THE PATIENT WILL CORRECT YOU...this will win you more points and ensures that you are involving pt in ur history.....iin the exam it may appear that you will fail sep if you do this ...but trust me it happened with me 5 times but i got high performance in sep....i was unable to understand one pt 3 times on a single point ..this infuriated the pt...i ws so scared...but it didnt bring any effect in the score and i dont know why...hehe
2- case: went good too....here i made a mistake,,when i was counselling the pt i asked him do u know what is sputum culture and the pt said no ...and i went in details of sputum culture and the pt gave expressions that i gave more detail of the technique then he could swallow...so be very simple in ur speech and use short sentences...like its a technique in which we examine the saliva for any abnormal growth of organisms(bacteria)...
3- the lady was vvvvvvvvvv rude
4- case: remember that now u wont have to bother putting the drapes on pt...they ll b already drapped...+ sp s will not be sitting always..they can be reading a novel,a paper,or walking around,,the good way to start such an encounter is to say' im sure mrs ---- i disturbed u..i see you were reading something....
5- phone encounter went good...
6-the patients now can now use some difficult words to check how u respond...like the pt asked me can i see a cryotherapist ...and i was like haaaaaaaaaaaaan?? who...sorry i dont what a cryotherapist is...just be sincere and tell what u dont know....
7- emergency case...went great...just practice hard to tell the pt that u will definitly give him something for the pain but to give the right meds u have to know whats going on...however u will limit the questions to the minimum
8- went good except that went i touched the abdomen..the lady jumped and said oh ur hands are cold...and i was like...oh oh oh im sooooooooooooooo sorryyyyyyyyyy...haha...this really made me feel i failed the encounter....just keep in mind that u will make hundreds of mistakes...but these are not mistakes this can happen in your real life encounters...the thing is to remain calm and show compassion...
9- went ok
10- telephone encounter went the best...
11,12 were just just fine
....PEARLS:
1-practice the 1st aid cases...take 8-10 mins for history...2 mins for the physical exam(be v focused) like in case of sob i just did chest and heart...nothing else,,in hematuria,,just examined abdomen,,,and check pallor in eyes...
2- after u r done with first aid cases...dont forget to practice the u world ...the video s of clinical examinations is also helpful
3- be compassionate,,speak slowly and clearly,,dont panic...
4- u will make mistakes but that mistake will fail u which is a true blunder....like being over confident,being rude,,not showing compassion,,,
* even if by mistake u called a female sp MR. this will not fail u.
5- this exam is very very easy to pass but at the same time its very very easy to fail it
6- speed up ur typing speed
7- answer the challenging questions very nicely.....tackle the issues in a diplomatic way...first aid will help u in this
8- practice live or on skype
9- its ok if you wash ur hands or wear gloves,,,if u take history while standing or u take a seat....this does nt effect ur scores and solely depends on ur choice
10- while washing hands or wearing gloves talk with the patient,,this is very nice time to build a good relation...u can ask...how do u spend ur day...what work do u do...
11- THE MOST IMPORTANT QUESTION IS TO ASK THE PATIENT ' WHAT DO U THINK IS THE REASON FOR THIS PROBLEM................TAKE MY WORDS...THIS IS THE MOST IMPORTANT AND USEFUL TIP
12- always do a good closure....summarize and in one or two sentences tell what tests u r going to do....
14- its best to spend full 15 mins in the room...in my case every time the proctor had to open the door...but at the same time i was telling the patient so take care ill see u after the test results etc.....
15- if you are finishing the case in 13 mins then its an alarming sign....it means that u were to0 fast and u did nt show compassion to the patient...
13- no matter how well ull perform u will always think u fail
ummm i had a lot in mind but at this time i just writting this....i hope this will be helpful.... BEST OF LUCK ...for the most insane and brutal exam...

hey!
CONGRATULATIONS on passing this exam.

I've started preparing today with FA and planning to buy csevideo after 1 read of FA.

Can you share what prep materials other than this you used? You also mentioned UW and i don't remember anyone recommending uw for CS. Is it good and how did it help you other than FA and csevideo?

Sorry for these many questions but i'm anxious to start.

@ cool_atomic...thanx and i wish u good luck for ur journey too.....and its ok u dont have to be sorry for asking questions ...i know and understand hw are u feeling ,,i have been thru...and believe me it was this forum that kept me alive esp in the post exam days.. anyways regarding ur questions...
i would recommend u world....just read at least the cases in it once ...esp when u will practice some cases from u world u will see how in some other ways the patient can present him/herself with the same dds....plus the histories in u world are a bit difficult so once u will do that ull feel a bit more comfortable...and + dont forget to do some cases like,,neck pain,picky eater ...and yes the mini cases are in first aid are the most imp...

and yes most importantly...the most useful thing was a video on youtube....esp for imgs...'USMLE Step 2 CS Videos - Persistent Cough by Rohan Khandelwal'



.......very very excellent...just follow the line of the lady in this video,,,and ull ace the exam im damn sure...

congratulations on passing the test and thank u so much for sharing it in such a detailed manner... i just have one qs tht as u mentioned the 1st aid mini cases...how did they help u? i mean they do help in makind dds ryt? but did u practice these cases with the partner too?

@ hareem..thanks and best wishes to u....well yes the mini cases will help u develop dds immediately...the patients will present to u with some what similar scenarios....and this is the most important thing that u develop 4-5 dds for a cc immediately on the door steps....your HPI should then revolve around only those 4 -5 dds...i.e max 7 -8 questions...then jump to PAMHUGSFOSS....u world will also help u to see how in different ways patients can present with the same cc....

Default MORE TIPS...( will be very useful)

HOW Should u perform in an encounter''''(these tips will apply to almost any case)

1- start: ' on the door way at once 4-5 dds should come to ur mind...and u should refresh in mind the questions for these dds..then open the door..start conversation like ..; "hello mr---- my name is dr ---- and i am the attending physcian on call today( u can use any title)..its nice to meet u...how can i help you....after the sp tells u the cc...say " oh i am really sorry to know about this,,,this mite have been really bothering you,(you really have to use ur face expressions at this time to show compassion and your concern), but i want to assure u that i will try my best to help u out...i am going to ask u some questions and then i will perform a brief physical exam after which i will be in a better position to say whats going on and what we can do for u...ok"

2- HPI: start with another open ended question...like..so mr--- u said that you had ------ can u tell me a little bit more about this''....then ask max 7-8 questions regarding the dds...USMLE really wants u to focus on the very common dds...during the HPI do summarize some points at least 2(this will ensure that u are actively involved,,and this will also give u a few seconds to think what other questions to ask)......also do at points show compassion with expressions and by saying...oh im sorry..oh

3- PAMHUGSFOSS: its v vvvv imp to do it...as many of ur dd s will be made while asking this....the key here is to use transitional sentences and let the patient know when u r changing the line of the questions ..for example before asking family history: ' say now i would like to ask you some questions about the other family members..is that fine'...before sexual say ' now i am going to ask u some questions related to ur sexual health...and i would like to mention here that every thing we r discussing will be kept confidential....'....
Note: if the patient says why you want to ask about my family member or my health or my social life....tell the patient politely..well mr--- i am really sorry to bother u with these questions however the problem that u r having can be related to any of the things that i am asking...so its v imp for us to talk about these things to help us find out what is exactly going on and what can be the reason for it..'

3- wash hands: tell the patient that u r done asking the questions and now you want to conduct a physical exam for which u have to wash ur hands....its very imp to keep talking to the pt while doing so...HERE THE MOST IMPORTANT QUESTION IS ( AND REALLY USMLE NOW ASSESS U REGARDING THIS POINT AND IT IS NOW ONE OF THE SUB COMPONENTS OF CIS)...; MR ----- BEFORE I MOVE FORWARD I WOULD LIKE TO KNOW IN WHAT WAY THIS PROBLEM HAS AFFECTED U.....??// once u will ask this question ...the patient will start telling u some many stuff as if he/she was waiting for u to ask this question....u will definitely remember me at that time and will send me great wishes in ur heart... ...here is also ur chance to improve ur cis score....relieve any stress the patient may have because of this problem.....

4- physical exam: b very focused...but start with hands and nails examination,,this will help u develop confidence...tell the patient what are u doing..why are u doing and what were the finds...like' i am examining the pallor in ur eyes as u r losing blood so this may tell the the severity of the blood loss.....+ well the eyes seem to be normal ( give a smile at this point)...
key point: dont spend time measuring pulse or bp if u think its imp to assess orthostatic vitals tell the pt that after u go out u will ask the nurse to measure the vitals in different position as they can be helpful..

5- summarize: at the end tell the pt that now u would like to summarize what u two just talked about in order to make sure i got things correctly...for example ; well mr ---- u said u had this,,,in addition u told me -------,, and then we talked about ur sexual ,social health etc( dont go in details)...is this right? would u like to add any thing to it...(its v imp to ask this as some patients will give u some imp point that u missed ....in my case two patients gave me very pertinent positive points when i asked this question)

6- closure: sample : ' mr----- i would now like to discuss my clinical impression,,it seems that the problem u r having can be becz of -------- ( if needed explain in very simple terms the dds) ...but to be sure i need to run a few tests like --------...after i get the result i will get back to u and then we can discuss further according to what we have on the results....ok do u have any question....ok then see u bye bye...'

THE PATIENT NOTE:
- write the dd s first then write the pertinet positive negative findings ( do check the vitals then can be listed as pertinent positive or negative points)....
- then write the work up
- then type the hpi...( a simple format is a pt M age -- with cc ----..there is associated -----(pertinent +) however there is no history of -----(pertinent -)..
- then wirte pamhugsfoss
- finally rite the physical exam...(see mnemonics from first aid )

i do hope that this post will be really fruitful...follow in every case this trend and i assure that By the Grace of God u will pass .. best wishes....

@..Amal1987...if you can afford...then sure go ahead..it will relieve ur anxiety...but if u cannot afford then no sweat...practice with a live partner for about 15 days...even if u attend the course ..live practice for 15 days ( average ) is a must...

First of all, Congratulations for getting through!

I have my exam in two weeks and I was using the First Aid 3rd edition. Only recently did I find out that the 4th one is out too. I do have the 3 added cases with me. Should I get the new book or will the 3rd edition and the 3 cases suffice?I am reading FA and Uworld and will be practising the cases from tomorrow onwards. Can you also tell me what is meants by 'PEARLS"?!

if you have the 3 added cases then its very fine to use 3rd edition...i did the same...

hi! congrats for passing! i also used FA 3rd edition, and got hold of the extra cases in the 4th edition. You mentioned that there isnt much change in the CIS component from the old CS. How can you ask the effect of the illness to the patient for a case which only presents an acute disease? like for example a transient infection or something? pls englighten me. thanks.

thanks....well i would again ask the patient ' well you said you recently started having these symptoms...in what way this has effected u/bought change in u/...the key is to ask some open ended question...the patient will definitely respond and you will score more points if you support the emotion at that time in accordance to the patient s response.......be gentle and show compassion...and do remain polite..........patients in cs with acute uri usually cough....and you should offer a glass of water, or a tissue paper...make sure that you specifically address any sign the patient shows like cough by saying..well i can see that you are coughing.....
