Default Breathlessness grading???

do we need to grade breathlessness in a patient like weather it is grade 1 ,2 or 3 according to medical research council classification or is there any other system of classification which i don't know about?
Do we really need to grade it?

I do not think you need to grade the breathlessness that way or another.
This exam is basic.
