Smile XPaezX Exam Experience, USMLE STEP 2CS

Hi all, just finished my exam and i'm in the lobby of my hotel waiting for my shuttle to take me to the airport.
Im in Houston BTW
Where to stay? The reality is that the baymont inn or park inn are the ****, cheap and awesome service+rooms. and are 200 meters away from the Test center.

What id I use?
first aid+Cse videos +Dr. Puma's Course+SP's I met here in the forum (You know who you are!)

The test center;

First of all it is amazing, its a tall bank building, the proctors are extremely nice and the SP's are OK, some are nice, some are weird; but I guess thats their role I guess.

How do I felt?
Overall I feel good, Only ran out of time 2 times and I was in the middle of my closure, managed to finish my PT note on time, and struggled in a couple of cases to finish the diagnosis, but i think that the preparation I had was enough..

Will I pass?
I dont know, probably yes, probably not.. we have to remember they have increased the exam difficulty and its tougher....

When will results come back? For people taking the exam during the end of january all throughout march I believe its gonna be until April the 24th!
FML right? almost 3 months of waiting.. ohh well...

Good luck to all people out there preparing for this exam.. I hope I dont have to take it again...

and also dont ask as I wont disclose the cases... I dont want to get into trouble

What is Dr. Puma's course? I've never hard about it! were all the cases familiar to you from what you've seen on CSE videos + first aid cases?

Yup they are similar, cse videos + First aid + good preparation and practice I think is enough. Its a guy that coaches you through skype, he is really awesome, and the price is really low compared to those big ass expensive courses.

Humm I just googled his name..so which package did you get? did you use it in the beginning of your preparation? was it useful mostly for what: PE? PN?

I used it towards the middle of my prep, he helps in everything man,I used the 6 day package

He sends you a patient note format in Word processor and then you complete that, then you send that to him and he shares his screen with you, telling you your mistakes

and what you have told about the exam experience is really helpful and thats actually good experience and you are definitely going to ''PASS'' with stars .....

I prepared for roughly 4 weeks of dedicated study time, I couldnt advise exactly how you can fit CS into CK because I didnt do that.. Im already done with step 1 and 2 CK

I would say that you take the course closer to your exam day, because everything he will tell you you will have it fresh, thats what I did, finished the course 1 week before my exam and then practiced with an SP with all the knowledge I acquired

How did you write relevant positive and negatives under each diagnosis and did you have 3 ddx for all the cases?

For example, patient has left knee pain

You put
Diagnosis 1: Left knee sprain
Hx findings
-Fell while running
-Swelling and redness
-stabbing pain 7/10

PE findings: No swelling or bruises
Tenderness on left knee
ROM limited due to pain
(-) Drawer sign

I had 3 DDx in all cases but one, the second one I was nervous and kind of had a brain fart, but on the rest I did have 3 DDx

Default My mistakes

All right guys, after some time has passed I started remembering stuff I did right and wrong during my exam.. will post it right now so that you can benefit from it.. and tell me what you think.

What I did wrong
  1. Got nervous in the first 2 cases, the second case I tried to show good empathy to my patient, she was overly serious and I couldnt steal a smile
  2. Wore gloves in all my patients, with 3 of them I removed their glasses and then forgot to change gloves.
  3. did extremely focused Physical exams + general exams. (I didnt do CV and Chest exam in all patients like almost everyone does...)
  4. did focused Neuro exams.
  5. During 1 of my encounters I already had finished my summary and closure and then the pt asked me a challenging question; while I was answering time ended and had to leave saying "got an emergency have to leave"
  6. In a phone encounter forgot to ask some past medical history questions.
  7. during some of my encounters I felt I was talking to fast.. eventho the patients didnt show any signs of struggling to understand me, 1 patient asked twice "what?" I do have a fairly good english, but nervousness + time pressure were against me I suppose.
  8. In some pts eventho I asked CAGE questionnaire i forgot to put in on the PT (FML, lol)

What I did right.
  1. Smiled, introduced myself by name, my role in the clinic and What I was gonna do.
  2. Asked for permission before every PE maneuver.
  3. Explained what I was doing and what I was finding.
  4. Asked for permission to write notes.
  5. Said sorry, this must be tough, I see several times.
  6. While wearing my gloves asked for anything else.. extra concerns etc, the superbowl game happened 1 day before, so I tried to have a conversation while putting my gloves, either that or tried to talk about the weather (a storm was gonna hit)
  7. Asked pretty much everything on first aid + extra ROS.
  8. ASked CAGE questionnaire on every patient that required it.
  9. Counseled on every patient about everything (sex, smoking, drugs, alcohol, depression)
  10. Congratulated patients on things they did right.
  11. Offered support and understanding several times during the encounter while trying not to sound like a repeating machine or a clown.
