Help How to deal with positive and negative findings in PN?

hello, guys..... really embarrassed with the new pn. plz let me know about this...
i'm wondering about real pn comments "enter the positive or negative findings from the history and the physical examination (if present) that support each diagnosis"

i'm bringing 2 FA doorway info(not in detail).... for your reference ---> abd pain // back pain

plz ignore the detail of the case. if neg rebound tenderness or obturator or psoas or rovsing in encounter, after getting out from the room, i may put just AGE at 1st d/d and maybe appendicitis at 2nd d/d. and then... can i write neg obtrator or psoas in p/e of AGE(1st d/d)?????? Real PN from official site definitely says "enter p/e that support ****each**** diagnosis. I'm really confused because neg rebound T or psoas is meaning just "not likely appendicitis". I think it is not supporting AGE specifically.

thinking in the same way of abd pain case,
if neg SLR in encounter, i'm gonna put muscle sprain at 1st d/d. At that time, can i write neg SLR to p/e of muscle sprain?. I also think that neg SLR is not supporting to decide 'muscle sprain' as 1st d/d, specifically.

Again, definitely official site pn says "write h/o and p/e that support ***each*** diagnosis. please let me know how to deal with this. thank you guys for reading this. plz help me with warm response.
