Help Seven Weird CS Confusions!

So these are few of the doubts I have been having.... Will be really thankful if someone can help me out...

1) Do we HAVE to give exactly 3 physical findings and 3 history findings for each differential?? Sometimes i have come across FA cases that cannot really have more than 2 exam and history findings. And some differentials especially dont have more than 1 finding and history. So what should we do?

2) Is it okay to type the PN in short sentences for eg: FA case 1. The lady with abd pain... Is this type of format ok?

HPI: Sharon Smith, 48 yo F c/o abdominal pain since 2 weeks. Epigastric pain, no radiation. Severity: 7/10. On and off and atleast one per day. Increased by fatty foods and relieved by milk, food and antacids. Getting worser. Nausea + and one episode of vomiting yesterday. Vomitus is sour yellow.

Now this is short sentences not well organised. I have been trying to practice and I am always running out of time except when I type this way. I have just provided the HPI here.

3) FA provides Exam findings under subheading as Chest, Abdomen, Heart etc. Is it OK if we type under RS, PA, CVS etc as that makes more sense.

4) Do we get a different form for the pt who walks in for a check up before joining work.

5) Would the sames be similar to the FA cases or are we going to have totally different cases. I know that about 5 are similar to FA. But by similar does that mean the same diagnosis or the same symptoms only?

6) Isnt it better to wear gloves than wash our hands. So why do a lot of people prefer washing hands?

7) If we elicit an exaggerated DTR but we arent really sure, is that NOT ok if we do it again? Coz I heard we must stay away from repeating any procedure twice.


I know this is a long set of QNs guys but kindly help me out with these coz I am really confused abt these.

That's OK. It's not a must to list 3.

Yeah. That's OK. You can use shortened format. In bullets or in short sentences, both acceptable.

Yes. You will get a special employment physical form.

It will never be exactly the same as FA. Perhaps similar presentation (symptoms leading to a diagnosis). So the idea is similar but it will never be exactly the same.
The NBME monitors the FA book and they make sure to make the cases different!

Wearing gloves the patient a sensation that there's something wrong with him (like he's infectious or something) but the NBME has specifically stated that there's no difference and you won't be penalized for using gloves.

If your SP moves his/her limb strongly after your hammer then he/she clearly wants to convey to you that the DTR is exaggerated. You don't need to do it again.
