Question All CAPS Patient Note

i have 2 questions for PN.

i heard that its ok if we write PN in all CAPS!! what about "workup". do we have to write that in all CAPS too if we are using that mode in rest of the PN?? e.g can we write HbsAg as HBSAG or igG ab as IGG AB???

2nd q:
is it ok if we write cardiac enzymes instead of writing CPK-MB, troponin and lipid profile and not LDL, HDL etc..and basic metabolic panel like in UW?

It is ok to write everything in CAPS, even in the workup part of the patient note.

Regarding your 2nd question.. you want to look as professional as you can, so First Aid and other sources clearly state that it is better to WRITE EVERYTHING, describe what you are ordering.. Not just put "cardiac enzymes" put everything.. not just put "Thyoid Hormones" but put TSH, T4, FT4 etc etc etc.

Remember that ICE and CIS have increased in difficulty, so its better to be extra safe.

i would not write cardiac enzymes rather than CPK-MB etc as it looks very generalized. regarding caps, you can absolutely do it but i wouldn't write HBSAG as it doesn't look professional. so with a very few exceptions, yes you can type in all caps.

thanx alot guys..what about metabolic panel coz it contain so many stuff..also stool analysis..it will take too much time if i start writing (stool ph,fat staining,osmotic gap,laxative screen)
