Smile FHS08 CS experience

I took my exam today as well. While PN was not an issue for me, I found it extremely difficult to cram the physical exams in the time limit we had. I tried to be as focused as I could, starting the physical exam with the complaint pt presented with. However, I forgot a couple. Didn't get to do DTR's for a fatigue case although I did palpate their thyroid. For hearing loss I forgot use the otoscope, I however did Rinne, Weber, whisper. I just feel no matter how much you practice, the real deal is different. I practically felt I was in an alternate universe where the 15 mins were passing a lot faster than they did at home practicing. Had a couple pts that were intimidating atleast for me. I got a little anxious but tried to overcome it. One thing I definitely did was counseled the heck out of every pt or at least to the best of my ability. For the ones who were doing everything right, I congratulated them for having a healthy lifestyle. For my first case, I ran out of time right around when I was about to finish the case so I hurried up and said nice to meet you. The next time that happened though I did the classic 'being paged' bit and even the SP laughed (she probably could see through the act)

I finished my PN within 6-8 mins for most cases and I had enough time to go back and review and make changes. Differential Diagnosis was not a problem for most of them, I didn't have all the boxes filled out for pertinent positive/negative.

I just know I tried my best. Even though I had all the necessary tools to pass this exam I did not get to apply all of them because of pressure, time limit, and anxiety to finish on time. This is all I can think of at this time, If you have any questions please let me know.

can you tell me do i need uw and cse video or just one of them in addition to FA.
BECAUSE I DO NOT WANT TO BE CONFUSED WITH MANY MATERIALS

I just focused on FA and Uworld. I didn't want to be overwhelmed with too many cases. I did watch physical exam videos over and over, that certainly helped. For my exam FA was more than enough, all the cases were pretty much by the book.

congratulations on being done.. I have a couple of basic questions,how much time did you take for preparation? also, how many hours did u put in each day? can it be done with 20 days of preparation starting right after giving step 2 ck?? what do u suggest..

I did about 4 weeks of hardcore prep between 4-6 hrs each day. Some days I had the energy to practice 12-15 cases, some days I could barely do 4. 20 days is definitely doable especially if you have CK out of the way.

heyy,, i just came out of the exam a few hours earlier ... and im trying not to think about it .. but i made some MAJOR blunders ... didnt counsel about a "certain social habit" coz i ran out of time ... missed to do fundoscopy in one case where it was necessary .. one patient i swear .. there was pain in kind of prohibited area and i wasnt sure if i was supposed to palpate or not so i didnt.. and i didnt make the patient walk in one case where i think it was necessary. not to mention missing questions here n there and PN blunders.OMG i hope these arent big enough to make me fail ((((

I know how you feel. I made a couple major mistakes as well. In one of the cases, the pt kept asking me what I thought it was and even though I had made the differential dx list on the door, I just blanked out infront of him and told him something really vague. Thankfully I remembered the differential dx for the PN note. One of the pts had knee pain, however, it was secondary to his primary complaint (and from what he told me the pain really wasn't an issue for him) regardless I should have had made him walk and check motor strength in LE but I didnt. However, I investigated his primary complaint thoroughly. I forgot to mention pelvic exam to a pt when it was warranted, even though I mentioned it in my note. For a pediatric case, I had nothing to counsel on but I did commend the father for being a concerned patient and how important it was to bring his child into the hospital so we can get him checked out. I had just enough time to ask only the focused questions, and I don't remember what I missed. Its all such a daze now, I really cant remember much of the details. I just now I gave it my best shot. I walked out of every room with a good idea of what the 3 differential dx were, and I did at least the focused exams necessary for those 3 diagnosis (for a couple blunders here and there)

Also, I felt the SP's I had were really laying on the acting pretty thick. One pt was in severe pain and she literally took 10-15 secs to answer each question. When I asked her to turn around so I can examine her she said 'hold on a min' and took over 30 secs to get in the proper position. I asked her twice if she needed help but I also didn't want to rush her at the expense of seeming hurried. I found that really annoying, a couple of those pts really sucked my time up. I understand they are trying to make it realistic but the exam is also timed and the time constraints are much severe than that of a regular clinic.

I think as long as you write fundoscopy in your workup, you should be fine. I really hope we all pass and do well.

Feel the same exact way...that 15 minutes goes by so damn quickly...I feel like my CIS and SEP were no problem, but im scared shitless about the ICE...many cases were very vague to me, and I made some blunders as in some examination maneuvers i did, I forgot to write on the PN due to time constraints.....sometimes I couldn't do a full exam due to time constraints, and some other times I left out some important history due to time constraints.... hate the waiting..

I know what you guys mean. I walked out of the exam with a huge smile on my face. And now when I sit down and think about the cases I am so nervous just :sorry: praying and begging to god that it'll all work out. I made one huge mistake of forgetting to write the work up for one of my case. The case was so heavy on PE findings, that I quicky jotted down my PE and DDx on my PN and moved onto HPI...and when the time was called and I scrolled down to click submit was when I noticed that I forgot to type my workup so REALLY nervous . Another mistake I made was the sp pt asked me a challenging question and instead of further questioning on it I just moved on. Another mistake I could think of was possibly not doing a PE for pain the pt was feeling, but I wasn't sure if I could touch there, but I think it wasn't as forbidden to not palpate there. I do agree with some of you guys that those 15 mins just zoom by, and I had a hard time typing some of the PEs I did......But what I do remember I did counsel where I thought counseling was needed, I tried my best to make the patient as comfortable as possible even if they were a bit intimidating, I made jokes when thought it was appropriate. I talked about my personal life, I showed empathy and apologized when I needed to. I summarized every single case, I closed every single case...but everything is so jumbled up in my head, I can't even remember if I did or did not do some of the things. The CIS and SPE part I think I did fine in those but the ICE oh man :sorry: please god save me! I pray that we all pass and put another step behind us.

It's been a roller-coaster of emotions since the exam last week. Everything you said is right on with how I feel. I can't remember the mistakes or the things or I did right anymore. It's all just a big blur. I am just waiting for that PASS to let me off the hook. Started CK prep again and I know I can't let this hang over my head otherwise I wouldn't be able to study. We did our best, just leave it for the best. We came this far for a reason, the rest will all fall into place as well.

Lol yup same exact story....hopefully what they say is true that building the relationship with the patient is by and far away the most important factor because i feel i did that fully.....even one patient who said shes never had a surgery, i said thats great me too!! she started cracking up...

For the most part though, all I can remember is crap i messed up on , this wait will be agonizing (dreamt about the result last 2-3 days) ah well though, finally starting back up for ck today should get my mind off of it

That's a nice thread.. I wrote CS a couple of weeks ago.. Dude, I'm so scared about the result.. I didn't think I had any issues regarding PE or the History, I was very detailed, but my time was short during the counseling part and I didn't do the counseling in 3 maybe 4 cases due to time constraints. Also, I didn't finish to write the PE, which I left to the end, during some cases, always missed something.. God help us!!!!

Hey guys- just an update to my threat. I passed CS with high performance in all three areas, ICE, CIS, SEP. So happy!!!! Now I can just focus on my CK, the next big thing.

You will, don't worry. I made plenty of errors on my exam. I didn't think I had a chance of even scoring in high performance. All the best to you

I didn't have that problem but if they do ask something you don't know you can tell them you are not certain at this moment, but you will check and get back to them right away. I had an SP who kept pressing me for the diagnosis, asking me what exactly do I think it is. I just gave him the general idea but also said I wouldn't know anything until I run some tests.
