how to write whisper test ( in a case of hearing loss ) in patient notes ?
if it is abnormal ?

Are you talking about the test where you speak from behind the patient with pt's eyes closed or rubbing your finger tips and asking the pt if he/she could hear it?
I feel that when you are documenting the PN you'll mention
under ENT; weber, rinne and AC>BC
and under the Cranial nerves 2-12; grossly intact except for decreased hearing
