Smile nishaa CS exam Experience

I got my results today and I PASSED Took exam May 13 in Houston .

MY PREPARATION:
Started preparing Mid April around 16th. I was already studying for CK then but I took a break to do CS. I studied FA twice, practiced cases everyday with online partner everyday. I managed to study about 3- 4 hours a day. I was really nervous about my typing, so I used to type PN everyday (1 or 2 ). Reached houston on May 10th.

Last few days before exam:

Practiced live for few hours with SP ( thanks @doxorubicin ) , read up FA mini cases.

Exam day:

Slept well previous night , (thanks to my experience with Step 1 when I could not sleep for 2 nights and went to exam messed up), I took zolpidem 5mg and went to bed. If you have issues with anxiety and insomnia please try taking one of these MAGIC PILLS

Had 12 cases, fairly simple, FA should cover for 95% of cases. So be thorough with FA cases , DD's for complaints, and what ever mnemonics you are used to.

I typed in all capitals, I was not confident switching between CAPS.

took a deep breath before I knocked on the door, had a smile as soon as I knocked the door. ( I used to practice this when I was alone at home knocking , smiling, introducing )

Things I did wrong:

1. I removed my gloves mid way between exam for one patient , don't know what I was thinking.
2. Forgot to counsel about smoking, sexual practices in one case
3. Left 1 DDx space blank in one case, my mind was blank
4. Forgot to do important test for the case.
5. Forgot asking few relevant questions in almost every case
6. Could not diagnose one case, made up something to just fill in the PN DD slots
7. Forgot to examine main system related to the complaint. ( I don't know what I was thinking)

THINGS I DID RIGHT:

1.knocked on door, smiled at every counter , shook hands
2.Put gloves for every case ( sometimes struggled to put them on but I always spoke to the SP every minute. Never leave a silent moment between)
3.Finished encounters on time.- As soon as you get the 10 min warning, you need to hurry up with the examination. If you are still in the History, end asking questions start the PE
4.Show empathy: Ask Sp if they are feeling comfortable, use magic words- I'm sorry to hear that, I understand this must be hard for you .... Keep a list of 5 sentences ready so that you can use them in your encounter.

My advice:

As everybody says, Practice practice practice. Live partner if possible but if you are preparing alone then practice with online SP.
Timing very important, time goes so fast in the exam before you know it, its over. keep practice time to finish up in 14 mins, so that in real exam you can wind up before 15 mins
Practice typing: everyday from the day you start your CS prep, 1 hour a day atleast, if you are a slow typer.
If you messed up one case in exam, don't think about it before you start next one. Clear your mind and tackle every case as a fresh new encounter
Keep calm, good night sleep very important for exam. Exam is not intimidating . Calm your nerves and go for it , you can ace it..

Congratulations for passing the exam.

Could you share with us how you explained your diagnosis and plan with the patient? Should it be as brief as possible or should it be given some time?

Better to be a bit elaborate , as it gives the patient a good impression about you. Closure is very important , but if you are running out of time better keep it brief and move on..

Okay this is what i told , " Thank you for cooperating during the physical exam Mr/Ms---- Based on your history and physical exam I think you are having ---- but it could also be --- or --- and for us to be sure we need to run few tests --- and -----. (here you could explain what the tests are in non medical words- for ex. if you say CT then you can also add it is an imaging study ) Once are results are in I will back to you and we can discuss how to proceed next. But I assure you what ever the results are we will do our best to get you back to your best. "

Thanks Totally don't remember what i used in the exam .. but let me try thinking of some lines..

1. Oh i'm so sorry to hear that... blah
2. I understand this must have been tough .. Blah
3. This must be hard for you to share but .. Blah ( use in abuse case if possible )
4. What ever the diagnosis is we will do our best to make you feel better (use in closure )
5. Oh I'm so sorry to hear that, my sympathies are with you ( if patient's mom or dad is dead use in Family History)
6. Are you feeling comfortable right now ( SP with pain )

Most important see the SP as a real patient, with real concerns and complaints. Just make them comfortable , 50% exam is the way you make an impression on your SP with your words and actions.

i was wondering if 19 days is enough for CS. i just finished CK and now preparing for CS..my exam is on July 8th enshalla

what do u think

please tell me did you summarize/narrate the patient history back to him? and btw, do we get the same computer to write PN everytime or we keep on changing computers??

Hi, since we are supposed to write only 3 differentials, are we also supposed to write DIAGNOSTIC STUDIES supporting those 3 differentials only ? or we should write more ?

i m to start with my prep for step 2 cs, but am not sure of what to read about. Could you plz guide me on what books should i refer to and if i would have to consutlt step 2 ck or step 1 books too

Yes I did summarize after the history but very quickly with only positive findings.Took maybe 30 - 45 sec to do it.

You change computers after each encounter. But exam is organised the staff always guide you what to do.

Make sure your top 3 tests are related to your DDs. You can add more, check out the usmle.org site to check the typing format. Its the same as in real exam

Hey parul, You don't need any book except FA for you can mnemonics in the forum that will help with writing your notes .You could subscribe to CSE videos. But most important thing is to pratice your cases. Good luck

Hi Nishaa,

many congratz to you on passing CS.. as i was reading your post i realized that i can relate to you very well in many ways.. i was preparing for CK uptil now but since my CS eligibility period will expire in September 2013 i better have to rush so i am starting my CS prep from scratch now.. I will be taking CS in 1st week of Sept.. I have two questions:

1) are 4 weeks enough for CS prep (I dont have live sp but will try to find someone on skype to practice).. I am an old IMG and have been out of practice for couple of years.. thinking of taking a live course but dates are not convinient

2) was it ok for you to conc. on CK after CS as i have read in other posts that its really hard to focus on anything esp exams after CS bec of post exam stress.. i am asking this bc i will be having my ck right in 3 weeks after CS..

1) 4 weeks is good enough to pass the exam, try to practice with a skype partner atleast as the exam prep involves more practice and less studying.
2) I was doing externship with my preparation and I did find it hard to focus back on CK as I felt I forgot many things. But its not impossible. I suggest in the 4 weeks of CS prep you try to solve 1 or 2 blocks of UW too. That way you will keep yourself well prepared.

Thanks Nishaa, i will start FA today.. do u recommend uworld for CS? plus since i have been out of practice for a while, do u think i should subscribe to online programmes.. whats ur opinion about csevideos? can you tell me how did u prepare for CS thanks
