Smile IMG, Passed CS on First attempt in just 18 Days.

Hi I am an IMG from India and gave my step-2 cs exam this may at Houston.
here is my experience...

Prep:
Prepared for only for 18 days as I don't had much time.
(1)Used First Aid as the main Reference.
(2)Watched UWORLD Videos for physical examination.
Did not do the cases from UWORLD as one of my friend who previously taken the CS had advised me that the format in both UWORLD and First Aid is very much different so I didn't wanted to get confused between them.
(3) Read the Neeraj CS Notes
(4) Read the digitaldoc's Step-2 CS note once.(Again easily available on web)

Study Partner:
I know this may sound crazy but I didn't practise with any study partner as i was not feeling comfortable in practising over skype and all of my other friends were busy in their step-3 preparation.

Stay in Houston:
I stayed in Hyatt North Houston it's just across the street from test centre. Though it's expensive (110.00 $ for 24 hours) I feel as they provide free airport shuttle and due to their location near test centre you will save plenty of money in transportation.

Test day:
arrived at test centre about an hour early but due to rules i was not permitted to enter the test centre before 30 minutes of exam so had to wait for 30 minutes at the ground floor which made me anxious about the exam.

Test
After the documentation and administrative procedure were complete we were taken into testing area.
Here is the list of mistakes which I made in each Patient.
1st Case: Pt suddenly stated crying and I tried to show some empathy towards her but judging from her later responses I think that was not enough. Also at the end of encounter she asked me a question and I was not able to counsel her properly and she said she is not satisfied.
2nd Case: read the doorway information in hurry and It was a pediatric case and instead of mother's name i addressed her with her child's name and she was like that's my child's name my name is...........
3rd case: Good no mistakes as far I remember
4th Case: again Good no mistakes as far I remember
5th Case: Forget to ask PMH, Surgical History, Family History, Habits and I feel this was really necessary in that case. I came out of that room about 5 min. early and when i started to write pt. Note i realised it but it was too late.
6th Case: Good no mistakes as far I remember
7th Case: I was suspecting substance abuse in that pt but he was not ready to give me history of that and was constantly trying to avoid me for that and have to counsel him lot to give me that particular history but in the end I forgot to counsel about leaving the substance abuse habit.
8th Case: Pt was very rude and I addressed him like hello Mr.............and he said its Dr.............not Mr.............and at one point he said why are you asking me so much question just refer me to a good doctor. also in this pt. note I forgot to add one most imp investigation which should be done.
9th Case: Good no mistakes as far I remember.
10th Case: The worst case of my exam the pt's accent was so weird I was not even able to understand her chief complain and even on doorway info they have not mentioned her chief complain. I asked about three times for her chief complain and was still not able to get an idea about what she is complaining. and in every question she was like denying everything I ask her. I was not able to find anything positive in her history or physical and when I came out of the room I had no idea of what that case was about still I tried to write the pt. note and it was the worst pt. note of my life.
11th Case: Good no mistakes as far I remember.
12th case: Good no mistakes as far I remember.

Things that I did Right:
(1) Always knocked on the door before entering.
(2) Tried to show empathy towards their pain.
(3) Always washed my hand before starting the physical exam.
(4) Helped them in seating up and lying down during physical exam.
(5) always helped in tying an untying their gown.
(6) Always asked their permission before starting the exam and always told them what I am going to do before stating any manoeuvre.
(7) Always asked about Social History except in 1 case and asked CAGE questionnaires when ever I felt necessary and done counselling for alcohol, smoking and substance abuse except 1 case.
(8) Always asked if they have any question or concern and tried to addressed them in best way that I can.
(9) Tried to use minimal medical terms and when ever it was needed I explained them in simplified form.
(10) Never tried to hurry the patient or interrupted them in between and I always had 1 to 2 min. left spare before the end of encounter.
(11) Before entering the room I always note down some mnemonic that help me remembering the things to ask and also from doorway info I made up my mind about diagnosis so that I can ask relevant questions.
(12) Never repeated painful procedure and always said sorry if my examination cased pain at any point.

After Exam:
Frankly speaking after Exam I was too much worried and thought I will Fail for sure and my dream of practising in USA will end here.:sorry:
At Last After 5 weeks of Long wait I received my result today and with god's mercy I passed my exam.:sorry:

I received borderline performance in ICE and CIS component but it doesn't matter now all that matter is I passed in First attempt.
I think After the revised criteria for scoring from Jan 2013 ICE and CIS is gone too much hard too score high performance so people out there preparing for step-2 CS please focus more on these components.
And please don't do the mistake of studying alone as me as I think if I had studied with a partner I would have scored more.

Hope this Helps to everyone who is going to take CS in near future.

I just read the whole book and was thinking I will ask this and this if I get this type of case.
but in real deal I was not able to remember half of the things and that's where the mnemonics came into play......that helped me a lot in remembering things.....

I have cleared step-1 and 2Ck but haven't posted the experience...sorry for that but search on this forum you will find plenty of it........

she was speaking English
but her speaking accent was weird
the way she was pronouncing the words I was not able to understand half of the words.
and no you will not be provided translator as she is speaking English.

thank you very for you answering me.
i have 1 more question
do you did heart and lung exam in all your cases or just do focused exam according to the case like as example if the case was tremor, knee pain, shoulder pain, wt gain....etc do i need to do heart and lung exam or do just focused exam. i know it's better to do them all but i don't want to run out of time for closure.

No I didn't do heart and lung examination in all cases I just did focused examination.
but again it depends on how much time you have left if you think you have more then enough time then you can do those examination as you can finish both examination in just 2 minutes......and yes always plan your time so you have some time left for closure it is one of the most imp. thing. Never forget it even if you think you have only enough time to do either closure or physical examination then go for closure.......
