Question Counseling of an HIV follow up case ?

Can anybody please tell me the bullet points to counsel an HIV pt and what statement should we use to tell his or her partner about his HIV status, esp if he or she is used to seek a new sexual partner after a short period of time ?

-"I suggest you to inform your sexual partners about your HIV status, since they should be screened and treated for it. If you don't, the hospital infectious department will contact your sexual partners, without disclosing your information, to come and have an HIV test. I am obligated by law to inform them. If you would like, you can come together with your partner to my office, and I can help in information sharing"

-"Please use condoms, otherwise you will infect your partners. Also, you will be at increased risk of acquiring other STD's"

-"Also, we have a really good support team here at our office who can help you, and educate you better about your condition. I can give you their information, and rest assured that I will be with you in every step of the way. Are you interested in meeting them?"

-"Please, be compliant to your medications, and have regular checkups."

I think that covers most of it. Correct me if I am wrong please.
