Question Mental Status Exam- to do or document?

Hey all
For psychotic and depressed patients do we just put mental status exam for work up or actually do the exam?
Anyone ever done it?

There isn't time for a full mini-mental. You can assess orientation, immediate memory (Giving 3 words, and asking about them 5 min later), and concentration (Spell "world" backwards) if you feel that is necessary. I think the First Aid says you can put a "full mini-mental" in the evaluations, and to me it makes sense.

i mean
the full mental status exam
which includes minimental
ie appearance behavior speech
mood affect thought process content
judgement insight etc
