Thumbs Up Umm

Count me in for the 21st. I can't believe the previous thread got more than 20,000 views. I wonder if every time I refreshed the thread was counted as one view. In that case I would be responsible for at least 200 of those 20,000. Anyhow, see you next Wednesday. Nice weekend!!

I heard its 65% ICE, 75% CIS and 90% in SEP. After all nobody can say how everything is graded in CS.

what exactly do u mean by this??
do we have to get this percentage in every patient interaction...or overall...
i.e. for passing in ice one has to be perfect in 9 pts of 12 for 75% evaluation...

Man... chill. I mean... If you can't manage the stress of this, how would you be able to manage more stressful situations during your clinical practice?

Don't get me wrong, I wish you the best. But I definitely think you should change the way you approach things.

All of you are correct. I do need to relax. Freaking out helps no one.

This exam however, is more than an exam. Its the culmination of 4 years of hard work. If it didn't go well, then I'm not sure what will happen next, because I'm an IMG. It's scary, to say the least.

Oh no...the countdown begins again. I seriously hope we all get our reports tomorrow, it's a horrible feeling to have to anticipate a possible failure. I just want to get over whatever the result is.

Is there any one else who's a little concerned about the rumors over massive numbers of AMGs failing? :/ It's scary that the results were officially delayed right after that rumor spread like wildfire!

About the AMGs failing? I assumed it was a rumor?! ohhh crap.

There is no evidence to substantiate the rumor btw. There has been no hint of any examinee getting their report last wednesday...

Smile Best wishes

OK guys!!

Best wishes for everyone :sorry:. I hope a big parade of "PASS" for tomorrow . Try to sleep. You know nothing will be released until around 9am ET tomorrow, so there's no point in staying awake the whole night, loging in every 5 min in OASIS, or refreshing this forum every 10 min before that time. Just CHILL OUT and hope for the best . Being worried or paranoic won't change tomorrow's results, so chill.

Suggestion: For this thread to be actually useful for everyone in the forum in the near future, along with your results try to recover your "These are my mistakes" posts from the other threads or at least include a link, and write a couple of lines about your preparation time and study resources. It's just a suggestion, it's up to you.

Ok, I already got my Zolpi... so good night.., see you tomorrow

I have spoken with a lot on this thread, and we have shared our mistakes. I know this won't help because anxiety is running high, but almost all of you are going to pass. They only way you wont, is if you have done more mistakes than you remembered/told me about (which is unlikely).

Relax. This time tomorrow, all of you will be celebrating. Say a quick prayer for me though, because as most of you know, I'm going to need all the help I can get.

Smile My experience

Hello!

My mistakes:

Exam: June XXth in the LA center

1. I forgot to use the leg rest in the first PE. I realized about it when I was writing my note and I paid extra attention to that in all the other PEs.
2. Run out of time in 1 case, so I missed the whole closing but I finished all the others.
3. I forgot to give counseling about smoking in the closing with 3 SPs.
4. I forgot to check lungs and did just full HEENT in a SP with cough. (I still cannot believe I did such mistake).
5. I forgot to put the table in 30º in my cardio cases (2).
6. I run out of time in 3 notes (so they were about 85-95% complete). In several notes I forgot to write some info or physical exam "findings" that I got in the encounter.
7. In about half of the cases I used only 2 of the 3 differentials. I think I missed the main diagnosis in 2 cases, but my differentials were pretty close.
8. I always completed the "History" part of the supporting statements, but honestly I had trouble filling the part of Physical findings, and in at least 4 cases I put "Normal physical examination", and I don't know if I'll get credit for that.

What I did:
Adequate greetings and introduction, going from open to closed ended questions, appropriate use of transitions, continuers and paraphrasing, good eye contact, showed empathy (at least I think so), reassurance of confidentiality when it was proper, established timeline of HPI, summarized the relevant facts of HPI, I can say I tackled every single challenging question ( I practiced a lot for those), I used globes, adequate draping technique (extra carefully with female SPs), I was careful with the potentially annoying maneuvers they mention in the video (ie. abd palpation), and closing including summary of physical, diagnosis, and plan.

Preparation:
5 weeks. I took Kaplan live course, used First Aid and CSE videos, and practice around 6 hours from Monday to Friday with study partners.

Status:
PASSED (High performance in SEP and CIS, borderline to high in ICE)
and ECFMG Certified.

Congratulations to all fellows who passed today!!

I second that. I did horrible mistakes and all I can think is fail during this whole 3 months wait. You just need to be confident and smile whatever happens to you during the exam. They're gonna stress you, give you a horrible look and frown upon your answers but remember they are just acting. They are just doing their job. They are doing what they are being asked. I find most SPs very nice. They won't help you with anything but they would smile at you and give you "don't worry, you'll be fine" look when you look into their eyes, smile and greet them.

and NEVER forget to ask the most important question at the end-"Is there anything else would you like to discuss which we haven't already? It will really help me to reach an accurate diagnosis in order to provide you with the best treatment for your condition ".
In two cases they gave me very important information which I missed earlier.
They will give you more info (and the diagnosis) if they like you & you ask for it (what do you think might be wrong with you?). One of them gave me the diagnosis on the plate just because I asked for it and she liked me.
In fact, in most cases (unless they are acting to be difficult) they gave me the whole history even without asking the questions since I made sure to be nice to them at the start of the encounter.
So, this test is NOT about any checklists but how you would treat a patient in the clinic. Be confident & nice to them and they will be nice to you.
Hope these tips help someone in future.

Congrats and thanks for that tip. So are you saying we should ask the pt "what do you think might be causing your [chief complaint]?"

Can anybody help interpret my performance bands. :P I'm curious - did I nearly fail in CIS? I think the band could mean that at best my CIS performance was average, and at worst, a near fail!

I was a little flustered the entire time and kept making little mistakes repeatedly, like asking close-ended questions (something I rarely do in real-life), never summarizing the H&P etc. However....I was still reasonably polite and empathetic ....

ICE: another surprise. I'm 99.99% sure all my diagnoses were correct! Here, I think my performance was pulled down by mediocre PE. I'm very self-critical and I'd still say my PNs were 8.5/10. I usually got the diagnosis from the history, and did extremely focused examinations to rule out or rule in differentials. I'm assuming that wasn't enough and I lost points in the SP's checklist!

I think I'll outline my experience and preparation in a little more detail to help future examinees. In retrospect, I could have gone about the exam a little differently..

Looks like you dodged a bullet there.
CIS has 40 points on checklist, so it looks like you consistently lost points for asking close ended questions not summarizing etc. What saved you was that you were polite and empathetic.
ICE is a different beast altogether and their points vary depending on the case.

That made me send out a fervent thanks to the universe. You are right. I have always been confident about my CIS skills from rotations and assessments at my hospital, but this exam is about doing things a certain way.

I should not have relied solely on First Aid I guess. Also didn't read the relevant official info manual on the CS and went into the exam with no real plan for acing the CIS. I just assumed I'd be okay!

Thanks for the input. I have a feeling saying things that come naturally (like 'Let me help you sort it out/I'm always available if you have questions after you leave' etc.) may have saved by behind.

Oh and one more thing I consistently did wrong. For each closure, I usually just gave just the most likely diagnosis. For e.g. "You probably have something called appendicitis. We'll have to run a few tests to make sure." It would have served me better to have said "You probably have x, but could also have y or z."

That's not wrong. That's how it is supposed to be done. So,don't be hard on yourself. It's the little things like leaning forward (to show empathy) when they are sad/anxious etc on which you lose points on.

Don't give up. You have better CK score than me and later grad year than me. Both Step 1 and Step 2 score are great. You have far more chance than me buddy.
