Question SPs name in the Patient notes

I didnt write any of the patient's name at the start of the patient notes during the 12 encounters..started with age and sex of the patient... of course I addressed the SPs with their names during the encounters. Can any one comment about the outcome??

That's the way it should be. You should not write the patient name in the PN.

but in the sample PN on site they did write the name? is it not the standard thing?
