First Aid First Aid mini cases

Started doing the mini cases but I am having trouble understanding it. For example: Pg 91

40 yr old F presents with occasional double vision and droopy eyelids at night with normalization by morning.

DD: Myasthenia gravis.Which is true based on the information given. Now my problem is how do these DD fits in?

Horner Syndrome
Multiple Sclerosis
Intracranial tumor compressing CN 3, 4, 6
ALS

How you guys are working on this? And it's not just this part, I am having trouble with whole mini cases so far.

Because of this, I am planning to do is taking these DDs that are grouped together, look at what they each have in common in terms of signs and symptoms. What do you think? Please let me as soon as possible.

Default MS and horners should be considered

you should consider raising MS and horners in this situation

i guess you can put more focus on MS especially if woman is in her 30s with diplopia tingling in her arms etc

I think D/Ds in FA are often approximative. They often include in the list diseases with very low likelihood of being the actual cause of patient's S/S. Personally I am working in this way: For each main category in the mni-cases, for instance "amenorrhea", I consider only the bold written diagnosis as the whole of D/Ds that I have to consider in let's say any amenorrhea case; then for each of these diagnosis I read the stem of the mini-case and try to memorize it as the information that should support the bold written diagnosis. I think this is the only way to give a meaning to the mini cases. Personally I don't like at all the D/Ds and the W/Us proposed on FA. Kaplan in much better.

ddx is better in other books i definitely do not recommend FA
