Question What should we write in PE findings for each differential?

Hello,

please help me

my exam is tomorrow

If we suspect appendicitis in a patient but on exam there is no tenderness on RLQ

so in the physical exam findings for appendicitis should we write no RLQ tenderness or should we leave it empty?

plz help it is urgent
