dont forget alleviating and exacerbating factors. exact location (left side of chest/retrosternal etc), intensity. Go by mnemonic LIQORAAA (loc, intensity, Quality, onset -progression, frequency, duration-, rad, allev, aggrav, associated sx (eg for chest pain: palpitations, sweating, nausea, SOB, leg swelling, hx of travel, cough, fever etc).

try to write the PN with that LIQORAAA as well. so you will minimize the things you left out.

your pertinent neg must be sufficient to r/o most of ddx.

On PMH may want to ask what was the dx.
