Default mistakes in CS.. SOO WORRIED!!!

i made many mistakes like forgot to ask some questions and forgot to do some system exam in some cases but some mistakes that r freaking me out are :

1) while interviewing SP i wasnt going in particular order(i mean my questions werent in group. i mixed up all questions) and questions were based on what SP was telling me.. i was being TOO MUCH specific..

2) in 1 case i asked few questions after closure!!! because i found the diagnosis after patient asked me a question

3) i was taking 11-12 mins only in each cases cuz i needed time to complete patient notes.

4) i didnt mention vital signs in all cases.. just 2-3 cases were i thought it is needed..

5) in some cases only while closure, i stammered. my speech wasnt so clear.

SOMEONE TELL ME ARE THOSE REALLY SERIOUS MISTAKES?>>?>?

Nothing serious.
1- The important thing is whether you gathered enough information and how you wrote it on your patient note
2- No problem, but you had to be sure that the sp hasnīt got any more questions before you left the room.
3- No problem, as long as you got the info.
4- Itīs a mistake, specially if the vitals were altered. You may loose some points on ICE
5- How often did the SP ask you to repeat your questions ? If not very often(less then 2 per sp), there is no problem. SEP is not very difficult to pass.

Hi. I too forgot to write down the vital signs for the first six patients in addition to not completing the differential diagnoses and treatment plan for one patient and still passed well. Relax.

i remember few more..

1) did not offer water when patient was coughing
2) did not say sorry when patient said his parents are dead
3) did not explain what tests i was gonna prescribe because i thought using medical terms are not good. i just said that m gonna run some tests.

What physical exam portions did you not do? I am worried as well because I forgot a neuro exam and msk exam when they were probably highly indicated.
