Default Minimental Exam in CS

Do we have to really count when we are doing a folestein MMSE exam and in the patient note and write the score the pt got out of 30.

You don't need to mention the score points in details unless it's absolutely necessary such as a case of Alzheimer disease.

In other neurological cases, for example, epilepsy, you don't need to count the socre.

You can write in the notes something like the following:
pt oriented to T/P/P, momory/recall/concentration WNL.
